Improvements in patient-reported outcomes, symptoms of depression and anxiety, and their association with clinical remission among patients with moderate-to-severe active early rheumatoid arthritis

Abstract: Among moderate-to-severe active early RA patients, clinical remission reduces symptoms of depression/anxiety, and independently improves PROs, thereby suppressing the negative impact of depression/anxiety on these measures.

“…Some studies show a positive correlation between depression and RA disease activity scores [5961] while others do not [32,46]. Regardless of acute disease activity measures, there is no doubt that limited function, as measured by the Health Assessment Questionnaire, is a strong predictor of depression in patients with RA [5,10,11,54,56,59,62,63]. Taking this one step further, loss of valued activities beyond functional decline has been shown to lead to depression [10,63].…”

“…It is a large (molecular weight of 150 000 Da) fusion protein that consists of extracellular region of human TNF-R2 fragment coupled to an Fc region of human IgG that binds to transmembrane TNF-a and acts as a decoy receptor (Mohler et al, 1993). Etanercept is clinically used in the treatment of RA and psoriasis, via subcutaneous administration (Hunt and Emery, 2013;Tyring et al, 2007), and reduces co-morbidities of depression and cognitive impairment (Kekow et al, 2011;Tyring et al, 2006).…”

“…Individuals with comorbid pain and depression experience higher morbidity and mortality relative to either condition alone (1)(2)(3)(4)(5)(6). In rheumatic conditions, pain of varying intensity and duration is one of the most significant symptoms for patients (7)(8)(9), and it has been estimated that 5-20% of patients have comorbid depression (5,10,11).…”

“…Role impairments associated with arthritis were largely confined to cases with comorbid mental disorders. Other studies have shown that pain and depression are more important predictors of disability and quality of life than radiographic joint damage and disease activity (5), and that arthritis patients with depression tend to be less compliant with treatment compared with arthritis patients without depression (6).…”
