Improvement of upper limb function with respect to urination techniques in quadriplegia

Abstract: A follow up study of urological management was carried out on 38 cervical spinal cord injury patients who were admitted to the Nagano Rehabilitation Centre Hospital and whose surgical reconstructions of their paralytic hands were performed since 1977. Self catheterisation was possible after reconstructive hand surgery in patients whose lowest functioning cord segment was C5-6. All of them had been unable to perform self catheterisation before surgery. Handling of instruments related to the act of urination was… Show more

“…Other authors have already found that patients need a strong key grip to perform ISC themselves. 15 We have found a statistical difference between the two groups for this outcome measure. None of the 'ISC-primary failure' patients could reach a key-grip value after an ULR of 0.5 kg (mean value being 0.28 kg), whereas 0.5 kg was the lowest score achieved in the ISC þ group (mean 1.8 kg).…”

“…13,14 To our knowledge, the contribution of ULR to the acquisition of ISC has not been specifically addressed in the literature in traumatic tetraplegics. 9,15 The objective of our study was to assess the efficacy of ULR protocols on acquisition of ISC in tetraplegic C5-C7 ASIA patients.…”

“…A previous study of ours 14 indicated that C6B1/ C6B2 and C6B3/C7A patients (Zancolli's classification; Table 3) could maintain ileal conduit formation independently after reconstructive hand surgery. This surgery is also recommended for tetraplegic people who need an ileal conduit for urological management.…”

“…However, many tetraplegic patients with inadequate hand function depend on family members or care attendants for assisted catheterization. 7,13 At the Nagano Rehabilitation Center, reconstructive hand surgery has been performed since 1977 14 in suitable tetraplegic patients to increase their independence in terms of ADL and urological management.…”
