Background: Burns, especially heat-related ones, are one of the most common injuries in work and daily life. If patients are not treated properly, burns can leave a lot of sequelae which would negatively affect not only daily activities, working ability but also aesthetics and psychology of the patients. Most topical treatments have been reported that they have some adverse effects and unintentionally prolong the length of hospital stay. Therefore, topical treating method is considered as an inefficient and uneconomical therapeutic approach. Hyperbaric oxygen therapy is a new treatment. It is able to help to relieve pain quickly, accelerate wound healing process, and reduce length of hospital stay which would effectively improve the burn treatment process. Objectives:To describe the clinical characteristics right at the time thermal burn patients were admitted to hospital and initial results of treating them by hyperbaric oxygen therapy. Materials and methods:Prospective and retrospective case series report. The study sample was the thermal burn patients who came to the Vietnam National Institute of Maritime Medicine for being treated by hyperbaric oxygen therapy in the period between 2018 and 2022. Results:of the 82 total thermal burned patients, the mean age was 48.4±19.5; the main cause of burns was boiled water which accounted for 74.4% of patients; 51.2% of them had been classified as 3rd degree burns; percentage of patients who had under 10% of total body surface area burned was 80.5%; 84.1% of patients had no pain on the 14th day; the mean length of hospital stay was 8.9±3.0 days which was significantly lower than the expected figures. Conclusions: Hyperbaric oxygen therapy is a new effective treatment for thermal burn patients with a variety of positive impacts on burned tissues. As a result, it should be scaled up, especially in health facilities which is specialized in burn treating.

Con la Oxigenoterapia Hiperbárica (OHB) se mantiene elevado el oxígeno en los tejidos y en el músculo por más de cuatro horas, aumentando la tensión de oxígeno en los fluidos corporales; mejorando la microcirculación, la actividad fagocítica de los glóbulos blancos; disminuyendo la agregación plaquetaria, la sobrecarga cardiaca; es bacteriostático bactericida y antifúngico, entre otras acciones. El Paciente ha ingresado a Tratamiento de Oxigenación Hiperbárica para tratar quemadura térmica en el 70% del pecho causada por agua hirviendo con quemaduras de 1º y 2º grado en una persona. El objetivo de la investigación fue cicatrizar la herida por quemadura térmica usando hiperoxia con vasoconstricción. El tipo de investigación fue experimental, con diseño Preexperimental. El enfoque de investigación que se usó fue mixto, ya que se realizaron 6 sesiones en cámara hiperbárica combinando productos tópicos y apósitos en un paciente; las técnicas utilizadas son: la observación y entrevista; instrumentos: cámara hiperbárica, oxímetro e historia clínica. Se logra hiperoxia con vasoconstricción y efecto Robin Hood en el periodo de seis semanas, acelerando la velocidad de cicatrización del tejido dérmico. El TOHB acelera la cicatrización y proliferación de células regeneradoras de tejidos epidérmicos con quemadura térmica.

Đặt vấn đề: Bỏng nhiệt là các thương tích của da hoặc các mô khác do tiếp xúc với nguồn nhiệt nóng hoặc lạnh. Tai nạn gây bỏng thường gặp trong lao động và sinh hoạt hàng ngày, chiếm 5-10% chấn thương ngoại khoa, là nguyên nhân thứ 9 gây nên gánh nặng bệnh tật và chấn thương toàn cầu. Tùy vào nguyên nhân gây bỏng, quá trình sơ cứu và mức độ bỏng mà mỗi bệnh nhân sẽ biểu hiện các đặc điểm lâm sàng, cận lâm sàng khác nhau, dựa vào đó sẽ có những hướng xử trí khác nhau. Mục tiêu nghiên cứu: Mô tả đặc điểm lâm sàng và cận lâm sàng ở bệnh nhân bỏng nhiệt. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang các đặc điểm lâm sàng và cận lâm sàng trên 82 bệnh nhân bỏng nhiệt đến điều trị tại Viện Y học biển Việt Nam từ tháng 01/2018-10/2022. Kết quả: Tuổi trung bình các đối tượng là 48,4±19,5; 51,2% đến viện trong 6 giờ đầu; 58,5% sơ cứu trước viện chưa đúng cách; 74,4% bỏng do nước sôi; 64,6% bệnh nhân bỏng phần thân dưới; 80,5% có diện tích bỏng <10%; 58,5% bệnh nhân đau mức độ nặng, chỉ số đau trung bình 7,0±1,7; nhóm bỏng độ III có tỷ lệ sơ cứu chưa tốt cao hơn, phần trăm diện tích bề mặt cơ thể bị bỏng và chỉ số đường máu cao hơn. Kết luận: Bỏng nhiệt xảy ra ở tất cả mọi đối tượng, nguyên nhân thường do nước sôi xảy ra trong tai nạn sinh hoạt, vị trí bỏng thường là phần thân dưới với mức độ trung bình, đa số bệnh nhân đau mức độ nặng. Nhóm bỏng độ III có tỷ lệ bệnh nhân sơ cứu vết thương chưa tốt, phần trăm diện tích bề mặt cơ thể bị bỏng và chỉ số đường huyết cao hơn so với nhóm bỏng độ II.
