Improvement of internal jugular vein cannulation using an ultrasound-guided technique

Abstract: Ultrasound guidance improved the success rate of jugular vein cannulation in ICU patients. Our results suggest that ultrasound guidance should be used when the internal jugular vein has not been successfully cannulated within 3 min by the external landmark-guided technique.

“…The range of experience, both with respect to the medical career and use of the intervention, differed greatly from study to study. Six studies described the operators as having up to 5 years postgraduate experience, 25,27,35,37,38,41 eight as having more than 5 years, 5,10,26,28,[32][33][34]39 and two as varying in experience. 6,31 Four trials did not record the career experience of the operator.…”

“…In three studies, 27,37,41 the operators were inexperienced both with ULD guidance and with the landmark method. In a further six studies, 6,25,26,34,35,38 the operators were inexperienced with ULD guidance and did not refer to their relative experience with the landmark method. Where this inexperience was defined, it was only in one study 35 where the operator had cannulated more than ten (but less than 30) patients with the ULD.…”

“…14 Another option to facilitate behavioral change would be to provide a portable ultrasound machine, as requested by our residents, which may reduce complication rates. 15,16 At our hospital, we decided to supplement hands-on training with expert bedside supervision during catheter insertion; the expert is provided through a procedure service that is led by hospitalists. The procedure service has a dedicated portable ultrasound machine to assist with internal jugular vein cannulation.…”

“…These include pneumothorax, hemothorax, air embolization, chylothorax, carotid or subclavian artery puncture and hemostasis control in children who have bleeding diatheses (Gauderer, 1992;Stovroff and Teague, 1998). Improvement of subclavian and internal jugular vein percutaneous cannulation using ultrasound-guided technique appears to distinguish arterial from venous flow, but it can be unsuccessful and has not been shown to reduce the incidence of complications (Lobe et al, 1993;Nosher et al, 1994;Slama et al, 1997). Albeit rare, lethal complications have also been described even if central catheter placement is performed as an elective procedure in expertised hands (Bagwell et al, 2000).…”
