Improved Results after Repair of Complete Atrioventricular Septal Defect

Abstract: By operating on the patients with complete AVSD earlier and not excluding patients with Down syndrome, recent results had definitely improved over the last decades. Despite this positive result, a high RPI exceeding 6 WU/m(2) still remains a risk factor for early mortality independent of early primary repair.

“…6 Early corrective cardiac surgeries are necessary to prevent irreversible pulmonary-vascular damage. 7 For patients presenting with unrepaired cyanotic congenital heart defects, prosthetic valves, or previous episodes of infective endocarditis (IE), as well as for recipients of cardiac transplantation who develop cardiac valvulopathy, the American Heart Association (AHA) 8 recommends the use of antibiotic prophylaxis ( Table 2) before dental procedures, which could cause transient bacteremias. According to the most recent AHA guidelines, 8 such procedures are those which involve manipulation of gingival tissue or the periapical region and perforation of the oral mucosa, such as during subgingival scaling, dental extractions, suture removal, rubber dam matrix placement, and placing of orthodontic bands.…”

“…Despite improvement in patient mortality, surgery has had a limited effect on the LAVVR after surgery in the last few years.Despite complete cleft closure, the prevalence of LVAVR is almost constant [13,34,35]. In North America and Europe, 9%-17% of patients with AVSD have significant left atrioventricular septal defect despite surgical repair [14,26].…”

“…Furthermore, the amount of residual defects such as mitral valve, shunt, and heart block failure were high [11]. However, improvements in precise preoperative diagnosis, better understanding of surgical anatomy, progression in surgical technique, protection and post-operative care (including treatment of pulmonary pressure events) contribute to increased survival rates and decreasing re-surgical intervention [12][13][14][15][16][17][18].…”
