Improved Outcomes of Thermal Ablation for Colorectal Liver Metastases: A 10-Year Analysis from the Prospective Amsterdam CORE Registry (AmCORE)

Abstract: Background To analyze long-term oncological outcomes of open and percutaneous thermal ablation in the treatment of patients with colorectal liver metastases (CRLM). Methods This assessment from a prospective, longitudinal tumor registry included 329 patients who underwent 541 procedures for 1350 CRLM from January 2010 to February 2021. Three cohorts were formed: 2010–2013 (129 procedures [53 percutaneous]), 2014–2017 (206 procedures [121 percutaneous]) and… Show more

“…38 Additionally, select literature has compared percutaneous approaches to open—not minimally invasive—surgical ablations, with obvious between-group differences for those cohorts. 39 Our group performs laparoscopic MWA on an outpatient basis, significantly reducing the cost differential between surgical and percutaneous approaches with low morbidity. One final point is that a substantial degree of ablation literature incorporates RFA; surgeons and interventional radiologists must reliably report their outcomes to ensure that prior studies and opinions remain supported by data now that MWA has supplanted RFA as the primary hepatic ablation modality.…”

“…The open or percutaneous approach to thermal ablation has been studied in the literature. Puijk et al., reported significantly improving liver tumor PFS following percutaneous ablation (2010-2013: 37.7%; 2014-2017: 69%; 2018-2021: 86.3%; P< 0.0001) whereas the PFS was stable following open ablations (2010-2013: 87.1%; 2014-2017: 92.7%; 2018-2021: 90.2%; P= 0.12) ( 56 ). The complications were less severe in percutaneous rather than open approach (2010-2013: P=0.69; 2014-2017: P= 0.129; 2018-2021: P= 0.02) ( 56 ).…”

The tissue damage secondary to ablation is low when compared to surgical resection, which is the most important requisite in patients with underlying liver disease or those who already had extensive liver resection ( 55 ).…”

“…If the CRLMs are primarily not eligible for local treatment, successful downstaging with induction chemotherapy can be achieved in up to 12.5% of patients. The 5-year OS increases up to 58% after local treatment of CRLMs in patients with liver only disease [ 5 , 10 , 11 , 12 , 13 , 14 , 15 ].…”
