Improved early prediction of pre‐eclampsia by combining second‐trimester maternal serum inhibin‐A and uterine artery Doppler

Abstract: Combination of second-trimester serum inhibin-A and uterine artery Doppler at 20 weeks improves the screening efficacy for the prediction of pre-eclampsia especially when this necessitates early delivery. This combination might form the basis of an effective and practical early screening test for the condition.

“…Cases of gestational hypertension without proteinuria are shown as circles, cases of preeclampsia as triangles. Prefumo It has been suggested that-in analogy to screening strategies for trisomy 21 (12)-the combination of multiple Doppler or maternal serum markers may allow better prediction capabilities (10,11,21,22). Combining different first-trimester Doppler and maternal serum markers could therefore improve our capabilities of identifying high-risk pregnancies early enough to reduce the risk of complications.…”

“…On the basis of published evidence [36] , if trials are performed to determine the real potential of therapies such as aspirin, calcium, or others, a screening test done during the fi rst half of pregnancy with a high sensitivity and high PPV ( 1 40%) and PLR ( 1 12) is essential. Therefore, if our results are confi rmed in larger studies, the combination of tHcy and uterine artery Doppler would appear to satisfy the criteria of an effective early method for predicting preeclampsia.…”

“…While low PAPP-A or hCG in the first trimester [31][32][33] or increased serum AFP, hCG, or inhibin-A in the second trimester [34][35][36] are associated with an increased risk of placentarelated diseases and early onset preeclampsia and IUGR, predictive values do not currently support routine use. Similarly, uterine artery Doppler can identify pregnancies at risk 37 but is not recommended for general application.…”
