Implications of different fluid overload definitions in pediatric stem cell transplant patients requiring continuous renal replacement therapy

Abstract: Our study is one of the first to compare different FO definitions and the impact on predicting outcomes. Our findings suggest that depending on the FO definition used, there is significant variability in the calculated %FO in PSCT patients, and this has important implications for clinical decision-making. Further studies are necessary to determine an optimal FO definition that is clinically relevant and predictive of important outcomes.

“…In people, FO is defined as a positive fluid balance diagnosed either by a positive weight gain during hospitalization or by a positive fluid score. In order to calculate a fluid score, a urinary catheter typically must be in place to measure urine output and can be calculated using several different equations . The development of FO usually occurs secondary to oliguric or anuric acute kidney injury, cardiac disease, or cardiorenal syndrome .…”

“…The increased incidence of renal dysfunction combined with the high fluid administration during the transplant results in an increased risk for fluid overload. Fluid overload is known to increase mortality in the general critical care population and specifically in the pediatric HCT recipients . However, very few studies have evaluated fluid overload in populations prior to PICU admission.…”

A singlecenter Canadian study reported weight gain to be a risk factor for PICU admission for both autologous and allogenic transplants.…”

“…Consensus definitions for fluid overload do not exist (38,39). Fluid overload may be defined as a positive fluid balance of at least 50-100 mL/kg on a given day.…”
