ABSTRACT. The purpose of this study was to investigate the changes in the humoral and cellular immunity of children with obstructive sleep apnea-hypopnea syndrome and hypertrophy of tonsils before and after plasma-mediated temperature-controlled radiofrequency ablation treatment. Fifty-seven children suffering from obstructive sleep apnea-hypopnea syndrome and with hypertrophy of tonsils were enrolled in this study. Thirty-seven children were grouped in the partial tonsillectomy group and 20, in the tonsillectomy group. The levels of CD3 + , CD4 + , CD8 + , and CD4 + /CD8 + were measured for cellular immunity, and the levels of IgG, IgA, and IgM were measured for humoral immunity. Blood samples were collected before and 1 and 3 months after the operation. The IgG, IgA, and IgM levels in the tonsillectomy group were significantly decreased 1 month after the operation, and recovered to the normal levels within 3 months of the operation (P < 0.05). However, the levels of IgG, IgA, and IgM in the partial tonsillectomy group decreased slightly, without a significant difference (P > 0.05). The cellular immunity of the 2 groups was not statistically different pre-and post-operation (P > 0.05). The results from the present study indicate that partial tonsillectomy by plasmamediated temperature-controlled radiofrequency ablation did not impact on the humoral and cellular immunity of children.

Background: Adenotonsillectomy is one of the commonest surgical procedures performed by the otolaryngologists. The immunological effects of adenotonsillectomy have not been studied extensively. Some researchers have found decreased immunoglobulin levels after adenotonsillectomy while others have not reported any significant changes. The present study aimed to evaluate the changes in humoral immunity in patients undergoing adenotonsillectomy before and after surgery as measured by serum immunoglobulin levels.Methods: The study designed was a prospective observational case-control study conducted at Yenepoya Medical College Hospital, a tertiary referral hospital, Mangalore, Karnataka. A total of 25 patients in the age range 5-15years who underwent adenotonsillectomy for chronic adenotonsillitis and 25 age and sex matched controls participated in the study. Serum immunoglobulin G, A and M (IgG, IgA and IgM) levels were estimated 24 hours prior and 4 weeks after adenotonsillectomy. The statistical analysis used was non-parametric tests.Results: Serum levels of IgG, A and M were found to be significantly higher in patients with chronic adenotonsillitis. The levels of all the three immunoglobulins decreased to normal one month post-operatively.Conclusions: Removal of infected tissue and chronic antigenic stimulation resulted in restoration of serum immunoglobulin levels to normal. While chronic adenotonsillitis contributed to changes in humoral immunity as reflected by higher serum immunoglobulin levels, adenotonsillectomy enabled restoration of the immunoglobulin levels to normal.

Objective: Given the conflicting evidence regarding adenotonsillectomy and the risk of upper airway infections (UAIs), including acute pharyngitis (APT), rhinosinusitis (ARS), and otitis media (AOM), we evaluated the risk of developing UAIs following adenotonsillectomy in a childhood population.Methods: In this population-based follow-up study, we used data from the National Health Insurance Service-National Sample Cohort. The adenotonsillectomy group (n = 2,377) included patients aged <10 years who underwent an adenotonsillectomy. For every patient with APT (n = 2,309), ARS (n = 2,308), and AOM (n = 2,207) who had an adenotonsillectomy, four participants were randomly selected for the control groups (n = 9,204, n = 9,196, and n = 8,788, respectively) using propensity score matching. The number of postoperative hospital visits for UAIs was recorded for 1 to 9 years, and the equivalence test was used to compare the number of visits between the adenotonsillectomy and control groups.Results: There were no significant differences in the incidence of APT, ARS, or AOM at the margin of equivalence of difference (−0.5 < 95% confidence interval of difference < 0.5) between the two groups from postoperative year 1 to year 9. The hospital visits for these diseases gradually decreased over time in both groups. There was a significant decrease in the number of visits for APT, ARS, and AOM in the 5-9 years age group when compared with those in the <4 years age group (P < .01).Conclusion: Our results suggest that adenotonsillectomy does not alter the frequency of UAIs in children.
