Implementing Technology-Supported Care for Depression and Alcohol Use Disorder in Primary Care in Colombia: Preliminary Findings

Abstract: Objective: Depression and alcohol use disorder are among the most common causes of disability and death worldwide. Health care systems are seeking ways to leverage technology to screen, evaluate, and treat these conditions, because workforce interventions alone, particularly in lowand middle-income countries, are insufficient. This article reports data from the first year of implementation of a technology-supported, systematic approach to identify and care for persons with these disorders in primary care in Co… Show more

“…We observed that women, patients with depression diagnosis, and those with any comorbidity were more likely to assess the ease of access to general healthcare as worse. First, this distribution reflects the actual demographic characteristics of the study sites population ( 12 , 25 ), suggesting that they remained as the more likely to use the services during the pandemic, even if access was difficult. Nevertheless, the poor perception of ease of access suggests an unmet healthcare need.…”

“…We will also adopt telemedicine approaches and introduce digital applications for implementing the clinical package where possible, including the provision of web-based training for PHC providers. Recent research from Colombia indicates that tablet-based depression and alcohol screening and clinical guidance can be successfully implemented in PHC settings, leading to increased rates of diagnoses, particularly for depressive disorders [ 68 ]. Similarly, an additional study in Peru showed that the use of screening app to identify at-risk patients followed by a motivational and reminder short message service (SMS) was feasible for implementation in PHC settings [ 69 ].…”

“…All participants were asked to use the Laddr mobile intervention daily during the 14-day study period. Informed by over 20 years of NIH-funded research and dozens of randomized trials (31)(32)(33)(34)(35), Laddr offers evidence-based health behavior change tools via an integrated platform to target a wide range of behavioral problems, including smoking, binge eating, alcohol use, other substance use, depression, panic, and anxiety. Each section of Laddr provides behavior-specific tools for activating behavior change, solving problems, overcoming obstacles to effective behavior change, and developing skills for maintenance of behavior change.…”
