Implementing RapidArc into clinical routine: A comprehensive program from machine QA to TPS validation and patient QA

Abstract: The all-in validation program allows a systematic approach in monitoring the different levels of RA treatments. With the systematic approach comes a better understanding of both the capabilities and the limits of the used solution. The program can be useful for implementation, but also for the validation of major upgrades.

“…This effectively filters out other features of the image to maximize the accurate detection of each of the dots. For each image (red and green), the image noise is reduced by applying a median filter with 5 × 5 pixels2 window size. Second, a 50% global threshold is used to identify the outline of the circle, which is subsequently used to locate the centroid of the circle.…”

“…Volumetric‐modulated arc therapy (VMAT) is a modern radiation treatment technique that allows a precise three‐dimensional (3D) radiation dose to be delivered as the gantry rotates through one or more arcs 1, 2, 3. Compared to intensity‐modulated radiation therapy (IMRT), where the gantry is static during dose delivery, VMAT offers more precise target coverage with lower monitor units (MU) 3, 4.…”

“…These three dynamic components (i.e., MLC, gantry angle, and dose rate) must remain synchronized throughout the treatment in order to deliver the intended dose to the planning target volume (PTV) and organs at risk (OAR). Due to this high level of complexity, there is a need for comprehensive and informative quality assurance (QA) techniques 4 , 5 …”

“…In this test, the leaves move in a single direction, a uniform leaf speed is used, and the leaf positions are not determined quantitatively. A set of different tests were proposed by van Esch et al; (5) however, these are very time‐consuming, involve film dosimetry, and have not gained widespread use. More recently, the Netherlands Commission on Radiation Dosimetry has proposed a more comprehensive quality assurance program for VMAT (13) .…”
