Implementing an Individual-Centric Discharge Process across Singapore Public Hospitals

Abstract: Singapore is one of the first known countries to implement an individual-centric discharge process across all public hospitals to manage frequent admissions—a perennial challenge for public healthcare, especially in an aging population.
