Implementation of basic quality control tests for malaria medicines in Amazon Basin countries: results for the 2005–2010 period

Abstract: BackgroundEnsuring the quality of malaria medicines is crucial in working toward malaria control and eventual elimination. Unlike other validated tests that can assess all critical quality attributes, which is the standard for determining the quality of medicines, basic tests are significantly less expensive, faster, and require less skilled labour; yet, these tests provide reproducible data and information on several critical quality attributes, such as identity, purity, content, and disintegration. “…Disintegration and dissolution have been reported in other studies as important indicators of poor drug quality [47], [51], [52]. A more comprehensive quality assessment, as recommended by the US Pharmacopoeia [50] and implemented in Amazon countries [47], should be considered in subsequent investigations. Systematic visual inspection of the packaging was not included as some samples were repacked from large jars in health facilities.…”

“…In addition, the people affected are often mobile and engaged in illegal activities, such as gold or gem mining, logging or fighting. In these areas there is no vector control, and ill persons often self medicate with erratic drug regimens, often using single drugs of variable quality, although for this the situation in South America seems to be better than in Southeast Asia [5,6]. …”

“…A fairly wide range of confirmatory tests were applied in the remaining 30/39 (77%) studies, although three studies did not specify the test that had been carried out [30, 34, 75]. Tests assessing both the physical and chemical properties of a medicine were undertaken.…”

“…Of the remaining 24 studies, 10 provided an account of limitations specifically relevant to the type of screening test used, of which, five studies included a discussion of the disparity between results from screening and confirmatory tests [5, 28, 29, 34, 65, 67, 69, 71, 74, 75]. …”
