Implementation of an institutional program to improve clinical and financial outcomes of mechanically ventilated patients: One-year outcomes and lessons learned*

Abstract: This institutional approach to the care of patients ventilated >3 days improved all clinical and financial outcomes of interest. To date, few similar initiatives have demonstrated similar results. The approach and lessons learned in this process improvement project may be helpful to other institutions attempting to improve outcomes in this vulnerable population.

“…These countries were selected for evaluation because study collaborators have previously described aspects of weaning processes through formal evaluation of protocols or automated weaning systems, 8,9 exploration of nurse roles and decision making, 12,13 description of nurses' education and training practices for mechanical ventilation, 14 and the study of an outcomes-managed approach to weaning. 15,16 We anticipate that the findings will provide relevant information for further work on weaning in these geographical areas.…”

“…The implementation of the CLABSI prevention bundle has significantly reduced CLABSI rates at multiple institutions (21,264). The use of a VAP prevention bundle has also led to significant reductions in VAP rates in ICU patients (41,272). In an effort to reduce SSIs, the CMS has instituted the Surgical Care Improvement Project (SCIP).…”

“…Some studies 20,21 point out the improvement in caloric intake and standardization of practice due to using nutrition-specific guidelines and protocols, although large gaps between recommendations from such guidelines and actual clinical practice have also been described. 5 Nevertheless, the development of nutrition-specific guidelines for critical care is an important step to providing synthesized, research-based recommendations that inform decisions at the bedside with the aim of improving nutritional intake for a group of patients at high risk for iatrogenic malnutrition.…”
