Implementation of a low titer group O whole blood program for a law enforcement tactical team

Abstract: The Texas Ranger Special Operations Group (SOG) performs high‐risk warrant service and responds to callouts for evolving kinetic situations and special missions as required. These operations may occur many hours from a trauma center. Fresh whole blood (FWB) transfusions may offer a stopgap for those who are critically injured. To make FWB transfusions a viable option, several steps must be implemented. The following lays out how the Texas Ranger SOG will implement and conduct FWB transfusions using low titer g… Show more

“…Based on hemovigilance data from many countries over decades, this is a safe procedure not increasing the risk of bacterial infections in the recipients 4,5 . Blood collected in a military walking blood bank is not LR 6 . When personnel, often with limited training, collect blood in a walking blood bank where hygiene procedures cannot be satisfactorily accomplished, there is a higher risk of bacterial contamination in the donated blood.…”

Reducing the risk of bacterial contamination is therefore of importance.…”

“…The risk can be minimized with plasma products containing low‐titer antibodies 19 . Moreover, the use of O whole blood with low‐titer antibodies for emergency/massive transfusion has been described 20‐22 . The use of group A plasma is suggested as an acceptable alternative to AB plasma 23,24 .…”
