Background Coaching has been shown to decrease physician burnout; however, coachee outcomes have been the focus. We report the impact of coaching on women-identifying surgeons who participated as coaches in a 9-month virtual program. Methods A coaching program was implemented in the Association of Women Surgeons (AWS) to determine the effects of coaching on well-being and burnout from 2018 to 2020. AWS members volunteered and completed training in professional development coaching. Pre- and post-study measures were assessed, and bivariate analysis performed based on burnout and professional fulfillment score. Results Seventy-five coaches participated; 57 completed both pre- and post-study surveys. There were no significant changes in burnout or professional fulfillment including the Positive Emotion, Engagement, Relationship, Meaning, and Accomplishment scale, hardiness, self-valuation, coping, gratitude, or intolerance of uncertainty scores from baseline to post-survey. On bivariate analysis, hardiness was associated with lower burnout throughout the duration of the program. Coaches with lower burnout at the end of the program met with their coachee more frequently than coaches with higher burnout [mean (SD) 3.95(2.16) versus 2.35(2.13) p = 0.0099]. Discussion Burnout and professional fulfillment demonstrated no change in women surgeons who participated as professional development coaches. Those with lower burnout and higher professional fulfillment at the end of the program were found to have higher hardiness, which may be worth future investigation. Conclusions Acquisition of coaching skills did not directly improve well-being in faculty who participated in a resident coaching program. Future studies would benefit from control groups and exploration of qualitative benefits of coaching.

BackgroundThe European Union faces severe and worsening personnel shortages in healthcare. Coaching has emerged as a human‐centred strategy to enhance sustainable employment and retention. While the number of efficacy studies on coaching continues to grow, knowledge about the barriers and facilitators to implementing coaching interventions among healthcare professionals (HCPs) remains scarce.ObjectivesThis systematic review aimed to describe common barriers and facilitators to the implementation of coaching interventions for HCPs.MethodsIn April 2023, five databases were searched for eligible articles. Barriers and facilitators were systematically identified and mapped onto the constructs of the Consolidated Framework for Implementation Research (CFIR). Directed content analysis yielded thematic areas and a reporting frequency.ResultsA total of thirty (n = 30) studies were included in this review, representing twenty‐five (n = 25) distinct coaching programmes. Implementation determinants were clustered under two CFIR domains: the Inner Setting (8 facilitators, 5 barriers) and Implementation Process (6 facilitators, 1 barrier). Barriers included (i) limited organisational capacity, (ii) lack of psychological safety, (iii) competing work demands, and (iv) insufficient leadership buy‐in, while facilitators were the (i) allocation of protected time for participants and coaches, (ii) promotion through opinion leaders, (iii) embeddedness in existing Continuous Professional Development programmes, and (iv) programme co‐creation.ConclusionThe findings of this study provide practical insights to guide the future implementation of coaching interventions at an organisational level. In particular, the identified barriers and facilitators suggest, for optimal efficacy and sustainment, coaching interventions must be implemented within a safe, supportive organisational climate.

Purpose The Professional Development Coaching Program (PDCP) is a physician coaching program founded on the principles of positive psychology that has been shown to improve burnout and well-being in residents. The experience of the physician faculty coaches is not well understood. We studied the impact of a longitudinal coaching intervention on the experience of coaches. Methods From 2017 to 2019, faculty from Surgery, Pediatrics, and Medicine at an academic medical center participated as coaches in the PDCP. Coaches underwent training in positive psychology and coaching skills and coached trainees for one or two years. Surveys were performed at baseline, after 1 year (EOY-1), and after 2 years of coaching (EOY-2). Outcomes include burnout and professional fulfillment (Stanford Professional Fulfillment Index), well-being (PERMA), and experience with coaching skills. Surveys from each timepoint were paired and analyzed through bivariate analyses and multivariate linear regression. Results Of the 136 coaches who participated, 44% submitted sufficient data for paired analysis. There was no change from pre-to post-in primary outcomes on bivariate analysis. On linear regression, burnout and PERMA scores declined from baseline but did not change from EOY-1 to EOY-2. Coaches reported increased coping skills on EOY surveys compared to baseline. Confidence with coping skills was associated with low burnout, high professional fulfillment, and high PERMA. Conclusions These findings create a profile of physician coaches and demonstrate both the benefits and changes in burnout and well-being that they experience. Larger studies with comparison groups can further explore the effects of coaching on the coach.
