Implantable Defibrillator Electrograms and Origin of Left Ventricular Impulses: An Analysis of Regionalization Ability and Visual Spatial Resolution

Abstract: ICD Electrograms and Origin of Impulses. Introduction:The implantable cardioverterdefibrillator (ICD) electrogram (EG) is a documentation of ventricular tachycardia. We prospectively analyzed EGs from ICD electrodes located at the right ventricle apex to establish (1) ability to regionalize origin of left ventricle (LV) impulses, and (2) spatial resolution to distinguish between paced sites. Methods and Results: LV electro-anatomic maps were generated in 15 patients. ICD-EGs were recorded during pacing from 22… Show more

“…Recently, Almendral and colleagues described the spatial resolution of pacemapping with respect to a recorded ICD electrogram . In that work, the spatial resolution of far‐field ICD electrogram matching a pacemap was determined to be 2 cm 2 , similar to what we have described in this study.…”

“…Stored VEGMs also can be used for distinguishing clinical VT from other induced VTs by using far‐field and near‐field VEGMs . Pace mapping analysis using intracardiac VEGMs with an ICD has been reported . This study focused on the VEGMs recorded via an ICD and evaluated whether VEGMs could expect the VT origin from the epicardia.…”

“…Few previous reports have analyzed the spatial resolution of IEGM morphological analysis to detect distinct exit sites of scar‐related VT . In a group of 20 patients with postinfarction VT, Yoshida et al evaluated the reliability of IEGM in distinguishing clinical from previously undocumented VT(s) .…”
