Impending central retinal vein occlusion in patient with idiopathic cutaneous leukocytoclastic vasculitis

Abstract: Purpose To report a case of impending central retinal vein occlusion (CRVO) associated with idiopathic cutaneous leukocytoclastic vasculitis (LCV) that demonstrated significant resolution following treatment with intravenous (IV) methylprednisolone. Observations A 27-year-old man presented to a tertiary Uveitis Clinic with a five-day history of blurry vision in the right eye (OD). He had a history of a purpuric rash and arthralgias five years ago and a biopsy-confirmed … Show more

“…Future studies should consider the in vivo assessment of vessel diameter by OCT-A and RH-OCT-A to improve the diagnosis of cutaneous vasculitis, an inflammation of the small blood vessels of the skin, which is classified by the size of vessels affected 46 . In addition, histopathology is limited to only providing morphological information, whereas OCT-A and RH-OCT-A have been shown to provide functional microvasculature information in response to local stimuli such as topical drugs, mechanical distortion, and temperature changes 16 , 47 .…”

“…Our finding of unchanged VT in any comparison supports the idea that RIPLs reflect chronic lesions similar to PAMM lesions. Prior case reports of PAMM have demonstrated that qualitative changes in tortuosity in PAMM generally follow a pattern of occasional acute increases in tortuosity21 22 that sometimes progressively worsen23 but that eventually improve 24…”

“…Fluorescein angiography displays no stops or delays in vein vascular flow when performed. 9 HFL hemorrhages in recent case series have shown spontaneous resolution within three months in almost half of the patients available, although it may cause ellipsoid zone disruption or residual hyperreflective bands crossing the vertical plane of the retina. 2 However, patients with decreased visual acuity affecting daily life activities or those with ellipsoid zone involvement may benefit from recombinant tissue plasminogen activator (r-TPA), sulfur hexafluoride (SF6) and anti-vascular endothelial growth factor (anti-VEGF), with or without vitrectomy, to displace the hemorrhages from the fovea.…”
