Impaired longitudinal deformation measured by speckle-tracking echocardiography in children with end-stage renal disease

Abstract: BackgroundLeft ventricular dysfunction is an important co-morbidity of end-stage renal disease (ESRD) and is associated with a poor prognosis in the adult population. In pediatric ESRD, left ventricular function is generally well preserved, but limited information is available on early changes in myocardial function. The aim of this study was to investigate myocardial mechanics in pediatric patients with ESRD using speckle-tracking echocardiography (STE).MethodsEchocardiographic studies, including M-mode, tiss… Show more

“…LV hypertrophy, which is observed even in early stage CKD [33], is mainly caused by hypertrophic responses in the mid-myocardial layers that are predominantly circumferentially orientated. This compensates for the reduction in longitudinal function and may explain preserved LVEF and GCS [41] in our cohort.…”

“…Intra-observer reproducibility for the offline strain measurements by the same pediatric cardiologist (IMK) has previously proven to be good. 28 Other articles also reveal a good reproducibility for the longitudinal LV strain measurements. 29…”

“…[24][25][26][27] Longitudinal LV strain measurements were performed based on previous reported protocols to measure end-systolic strain. 28 Manual off-line tracing of the endocardial LV border was done during end-systole for the following six segments: apical septum, mid septum, basal septum and apical lateral, mid lateral, basal lateral. Segments were automatically approved or rejected by the program and subsequently needed manual approval.…”

“…The development of two-dimensional (2D) speckle tracking echocardiography (STE) has facilitated the simple and angle-independent measurement of LV myocardial deformation in the longitudinal, circumferential, and radial directions, which can be used to detect subclinical uremic cardiomyopathy when LVEF is normal [7, 8]. LV strain has been proposed as a novel indication of systolic function.…”
