Impacts of Geriatric Evaluation and Management Programs on Defined Outcomes: Overview of the Evidence

Abstract: Comprehensive geriatric assessment is a technique for multidimensional diagnosis of frail elderly people with the purpose of planning and/or delivering medical, psychosocial, and rehabilitative care. When comprehensive geriatric assessment is coupled with some therapy, then the term geriatric evaluation and management (GEM) will be used. Following a brief history of comprehensive geriatric assessment, we describe the varied patterns of GEM program organization and review the literature of studies examining GEM… Show more

“…A GA is defined as a multidimensional, interdisciplinary diagnostic process focusing on determining an older person's medical, psychosocial, and functional capabilities to develop a coordinated and integrated plan for treatment and long-term follow-up (10). Such a GA has been shown to successfully identify patients at risk for poor outcome in geriatric oncology (11) and improve outcomes in older patients admitted to the emergency department (9), and it is increasingly recommended as being the standard of care in the treatment decisionmaking process in elderly patients (8).…”

“…To assess the profile of each individual patient, a multidimensional geriatric assessment (GA) [2,3].The GA produces an inventory of health problems and an assessment of physical, psychosocial, and functional capabilities [4].The GA has proved useful for characterizing health and functional impairments potentially associated with oncological outcomes [2,[5][6][7]. Nevertheless, it requires considerable time and human resources and is actually not required in all patients [6].…”

“…Aside from the tool's ability to identify those at risk of future fall, it also identifies a cohort at risk of functional decline and reduced quality of life, and so may be useful method of targeting older people in the primary care setting for interventions aimed at maintaining their independence. Such interventions, generally known as comprehensive geriatric assessment, have shown benefit in primary care settings for frail older people with a variety of geriatric syndromes, not just falls [332][333][334] .…”
