Odontomas are categorised into mixed odontogenic tumor that originates ecto-mesenchymally and made up of mineralised tissue. Hamartoma is the most frequent benign tumour with an odontogenic aetiology. It is distinguished by its sluggish growth and lack of aggression. "Erupted odontomas" are those that erupt into the oral cavity and are uncommon. They are usually noticed in regular radiography and can be caused by a variety of factors, including trauma, genetic mutations, or infection. They are categorised into complex and compound types. Erupted Complex Odontomas are rarely ensued into the oral cavity. The present case report is of such unusual erupted complex odontoma, where 17-year-old male patient reported with history of pus discharge from the left side of the face. This report is of clinical importance because it was associated with impacted tooth & agenesis of adjacent molar tooth which is usually rare. We provide clinical and radiographic findings as well as treatment for a unique instance of a massive erupted complicated odontoma in the maxilla.

Objective: In this article, we intend to report the management and the outcome of the surgical treatment of an unusual case of impacted mandibular premolar. Method: A 57-year-old man with an unremarkable medical history was referred by her general dentist for the management of his impacted mandibular left second premolar. The patient complained of an annoying feeling in correspondence of the lingual side of the left mandibular body. Results: Surgical intervention of the impacted mandibular left second premolar was decided in agreement with the patient under local anesthesia. On the 10th day after intervention, the patient complained second premolar was decided in agreement with the patient under local anesthesia. On the 10th day after intervention, the patient complained no pain, and he showed neither paresthesia nor anesthesia in the region of the chin. No sign of infection was observed. The patient was clinically monitored during the year after the surgery. Six months follow up postoperative panoramic radiograph confirmed a good healing. Conclusion:When surgery is the chosen option, a great attention should be paid not to damage the inferior alveolar nerve. The indication for the removal should be carefully assessed in adult patients.

Introdução: Tumores odontogênicos são classificados de acordo com a sua origem epitelial e/ou ectomesenquimal. O Odontoma,possui incidência de 22 a 67%, assim sendo o tumor odontogênico com maior frequência relatado na cavidade oral. O Odontoma Composto é um tumor benigno geralmente assintomático que se origina da proliferação exagerada da lâmina dura. Acomente pacientes de todas as fixas etárias e mais em indivíduos do sexo masculino do que feminino. O diagnóstico do Odontoma Composto e Complexo é confirmado por meio da avaliação de exames de imagem e exame clínico. O tratamento é cirúrgico e, em muitos casos, é de fácil remoção. Entre as consequências ocasionadas pelos Odontomas, há a retenção de dentes permanentes. Relato de Caso: Paciente do sexo masculino, 20 anos, feoderma, compareceu a clínica particular localizada no município de Vitória da Conquista, com objetivo de realizar tratamento ortodôntico. Após o exame físico intraoral, foi solicitado como primeiro exame de imagem, a radiografia panorâmica, ao avaliar este exame, na região anterior da mandíbula foi observado uma imagem radiopaca similar a odontoma composto, a partir disto foi necessário solicitar a tomografia computadorizada cone beam entre os elementos dentários 42 e 43. Após análise tomográfica, foi levantado a hipótese diagnóstica de odontoma composto sem associação a impactação de dentes permanentes. O tratamento da lesão proposto ao paciente foi a exérese da lesão com a preservação dos tecidos circunjacentes. Conclusão: A enucleação da lesão com a preservação dos tecidos duro e mole é importante para a manutenção da função mastigatória, estética e de fonação para o paciente. Isso é estabelecido com a realização dos exames clínicos associado aos exames de imagem, que contribuem para um diagnóstico com maior confiabilidade, planejamento cirúrgico e/ou ortodôntico e a previsibilidade de um prognóstico favorável. Ademais, cabe ao cirurgião-dentista solicitar exames complementares para a realização da consulta odontológica, pois lesões em cavidade oral podem não ser observadas ao exame físico intra e extraoral, mas sim em exames de imagem.
