Objective:The objective of this study is to systematically analyze the effects of diabetes mellitus/hyperglycemia (DM/HG) on peri-implant biomarkers and clinical and radiographic outcomes in patients undergoing implant treatment.Materials and Methods: PubMed, Embase, and the Cochrane Central Register of Controlled Trials databases were searched up to March 2022. Studies reporting on peri-implant biomarkers, and clinical and radiographic outcomes in patients with different glycemic status levels (HbA1c) were included. Several analyses were conducted, such as meta-analysis, sensitivity analysis, dose-response meta-analysis (DRMA), and robust error meta-regression (REMR).Results: A total of 10 studies and 634 participants were included for analysis, with a maximum follow-up of six years in function. The level of advanced glycation end products (AGEs) in peri-implant crevicular fluid (PICF) was significantly higher in the DM/ HG group than in the healthy group (p < .01). Subgroup analyses showed that a set of negative regulators of bone metabolism (including IL-6, TNFα, IL-8, and RANKL) were significantly higher in the DM/HG group (p = .01). Implant survival rate (100%) was not compromised in patients with an HbA1c level less than 10%. Outcomes of implant stability quotient (ISQ) values, bleeding on probing rate (BOP%), probing depth (PD), and marginal bone loss (MBL) in the DM/HG group were significantly worse than those in the healthy group (p = .04, <0.01, 0.01, <0.01, respectively). DRMA results showed AGE accumulation in PICF, PD, and MBL worsened in a dose-response dependent manner with elevated HbA1c levels (<6%, 6-8%, >8%; p = .04, 0.02, <0.01, respectively).Conclusions: DM/HG can upregulate negative regulators of bone metabolism and compromise peri-implant health. In addition, there are dose-response relationships between HbA1c levels and AGE accumulation in PICF, PD, and MBL.

The failure rate of dental implantation in patients with well-controlled type 2 diabetes mellitus (T2DM) is higher than that in non-diabetic patients. This due, in part, to the impaired function of bone marrow mesenchymal stem cells (BMSCs) from the jawbone marrow of T2DM patients (DM-BMSCs), limiting implant osseointegration. RNA N6-methyladenine (m6A) is important for BMSC function and diabetes regulation. However, it remains unclear how to best regulate m6A modifications in DM-BMSCs to enhance function. Based on the “m6A site methylation stoichiometry” of m6A single nucleotide arrays, we identified 834 differential m6A-methylated genes in DM-BMSCs compared with normal-BMSCs (N-BMSCs), including 43 and 790 m6A hypermethylated and hypomethylated genes, respectively, and 1 gene containing hyper- and hypomethylated m6A sites. Differential m6A hypermethylated sites were primarily distributed in the coding sequence, while hypomethylated sites were mainly in the 3′-untranslated region. The largest and smallest proportions of m6A-methylated genes were on chromosome 1 and 21, respectively. MazF-PCR and real-time RT-PCR results for the validation of erythrocyte membrane protein band 4.1 like 3, activity-dependent neuroprotector homeobox (ADNP), growth differentiation factor 11 (GDF11), and regulator of G protein signalling 2 agree with m6A single nucleotide array results; ADNP and GDF11 mRNA expression decreased in DM-BMSCs. Furthermore, gene ontology and Kyoto Encyclopedia of Genes and Genomes analyses suggested that most of these genes were enriched in metabolic processes. This study reveals the differential m6A sites of DM-BMSCs compared with N-BMSCs and identifies candidate target genes to enhance BMSC function and improve implantation success in T2DM patients.

Introduction: Diabetes mellitus is a chronic metabolic disorder leading to hyperglycemia, which causes various complications due to vasculopathy. Adequate dental rehabilitation including dental implants plays a key role in promoting the eating habits of diabetics and better metabolic control. Aim: We performed a systematic literature review to investigate the effect of diabetes mellitus on the stabilization and osseointegration of dental implants. Methods: We searched PubMed and Scopus until October 2021. We used the following inclusion criteria: (i) study population included diabetics type I or II, (ii) outcomes were the dental implant failure or resonance frequency analysis, (iii) studies that investigate the effect of diabetes mellitus on the stabilization and osseointegration of dental implants, (iv) studies that were published in English, (v) studies that were published in journals with peer review system, (vi) studies including humans and not animals, (vii) studies that compared diabetics with non-diabetics, and (ix) quantitative studies. Results: Twenty-nine studies met the inclusion criteria. Regarding implant failure, four studies found statistically significant more frequent implant failure in diabetics, while five studies found that implant failure was more frequent in diabetics but was not statistically significant. In contrast, ten studies found that implant failure was more frequent in non-diabetics but was not statistically significant. In addition, seven studies found that all diabetics and non-diabetics retained their implant during the study. In six studies that performed the resonance frequency analysis, no statistically significant difference was found between diabetics and non-diabetics. In three studies, the mean value of the implant stability quotient increased statistically significant in non-diabetics, while in three studies the mean value of the implant stability quotient increased statistically significant in diabetics. Conclusions: The results of this review suggest that implant failure is not higher for diabetics than for non-diabetics. Diabetics seem to be able to achieve a rate of dental implants survival like that of non-diabetics. With regards to the resonance frequency analysis, no difference is found between diabetics and non-diabetics.
