Barthel’s Index: A Better Predictor for COVID-19 Mortality Than Comorbidities

BackgroundThe most consistent identified mortality determinants for the new coronavirus 2019 infection are aging, male sex, cardiovascular/respiratory diseases, and cancer. They were determined from heterogeneous cohorts that included patients with different disease severity and previous condition.The main goal of our study was to test if activities of daily living (ADL) dependence measured by Barthel's index could be a predictor for COVID-19 mortality. MethodsProspective cohort study with a consecutive sample of 340 COVID-19 patients representing patients from all over the Northern region of Portugal from 10/2020 to 03/2021. Mortality risk factors were determined controlling for demographics, activities of daily living (ADL) dependence, admission time, comorbidities, clinical manifestations, and delay-time for diagnosis. Central tendency measures were used to analyze continuous variables and absolute numbers (proportions) for categorical variables. For univariable analysis we used T-Test/Chi-Square/Fisher Exact Test as appropriate (α=0.05). Multivariable analysis was performed using logistic regression. Statistical software: IBM®SPSS®27. ResultsThe cohort included 340 patients (55.3% females, mean age 80.6±11.0) with a mortality rate of 19.7%. Aging, ADL dependence, pneumonia and dementia were associated with mortality and dyslipidemia and obesity with survival in univariate analysis. In multivariable analysis dyslipidemia (OR=0.35, 95%CI:0.17-0.71) was independently associated with survival. Age ≥ 86-year-old (pooled OR 2.239, 95% CI:1.100-4.559), pneumonia (pooled OR 3.00, 95% CI: 1.362-6.606), and ADL dependence (pooled OR 6.296, 95% CI: 1.795-22.088) were significantly related to mortality (ROC AUC=82.1% (p<0.0001)). ConclusionADL dependence, aging and pneumonia are the 3 main COVID-19 mortality predictors among an elderly population.

ObjectivesPatients with a history of active malignancy were initially thought to be at a higher risk of having COVID-19, although available data are conflicting due to economic stress, malnutrition, fear of hospitalization or treatment discontinuation. A cohort-based study was undertaken in Indian regional cancer centre to understand cancer-covid link in patients.Study DesignA total of 1565 asymptomatic patients were admitted based on thermal screening and evaluation from the screening form. The COVID 19 has been checked by RT-PCR method and the COVID 19 positive patients were transferred to government allocated COVID 19 hospital. The COVID 19 negative patients were transferred to general ward for further cancer treatment.MethodPost COVID 19 testing, positive patients were transferred to COVID hospital and their outcomes were analyzed and correlated with patient’s age gender and cancer stages.ResultOut of 1565 patients, 54 patients (3.4%) tested positive. Most of the patients are in 45-59 years age group. As female patients admitted were more in number than males, so predominance of disease is higher in female. 3 patients were symptomatic after admission and 2 were severe and were admitted to the ICU with ventilations. 8 patients died in Cancer and one patient died in COVID 19.ConclusionsAs only 3.4% patients tested positive and only one patient out of 54 had died, so cancer is found not to be a comorbid condition towards COVID 19 patients in the Indian population studied.

Patient‐reported disruptions to cancer care during the COVID‐19 pandemic: A national cross‐sectional study

Background The aim of this study is to evaluate the extent and associations with patient‐reported disruptions to cancer treatment and cancer‐related care during the COVID‐19 pandemic utilizing nationally representative data. Methods This analysis uses data from the 2020 National Health Interview Survey (NHIS), an annual, cross‐sectional survey of US adults. Adults (age >18) who reported requiring current cancer treatment or other cancer‐related medical care in the second half of 2020 were included. Estimated proportions of patients with self‐reported changes, delays, or cancelations to cancer treatment or other cancer care due to the COVID‐19 pandemic were calculated using sampling weights and associations with sociodemographic and other health‐related variables were analyzed. Results In total, 574 (sample‐weighted estimate of 2,867,326) adults reported requiring cancer treatment and/or other cancer care since the start of the COVID‐19 pandemic. An estimated 32.1% reported any change, delay, or cancelation. On sample‐weighted univariable analysis, patients who were younger, female, had one or fewer comorbidities, and uninsured were significantly more likely to report disruptions. On sample‐weighted, multivariable analysis, patients who were younger and female remained significant predictors. Nearly 90% of patients included in the study reported virtual appointment use. Patients reporting disruptions were also significantly more likely to report feelings of anxiety. Conclusions An estimated 1/3 of patients experienced disruptions to cancer care due to the COVID‐19 pandemic. Patients experiencing disruptions in care were more likely to be female or younger which may reflect risk stratification strategies in the early stages of the pandemic, and also had higher rates of anxiety. The longitudinal impact of these disruptions on outcomes merits further study.
