General Medical Sciences, currently funds 40 programs involving 45 degreegranting institutions. 2 Approximately 75 US allopathic medical schools offer MD/PhD degree programs not supported by MSTP grants. 2 Historically, the primary intent of these joint MD/ PhD degree programs, regardless of their funding mechanisms, has been to produce highly trained physicianscientists who will engage in biomedical science research careers.The medical education environment in which MD/PhD programs are offered has changed markedly in the past 15 years. The demographic characteristics of medical school graduates have changed; most notably, the proportions of women among matriculating medical student populations have steadily increased and are now approaching parity. 3 Specialty choice preferences for graduate medical education (GME) among recent US allopathic medical graduates have shifted away from the generalist specialties of family medicine, internal medicine, and pediatrics as greater proportions of gradu-ates have chosen to pursue GME in "controllable lifestyle" specialties. 4,5 In addition, professional setting options for physicians have expanded well beyond the dichotomy of academic medicine or private clinical practice. 6 Moreover, US allopathic medical graduates are faced with steadily increasing debt loads. 7 The extent to which the characteristics and professional plans of recent US allopathic medical graduates may differ between MD/PhD program gradu-ates and other MD program graduates has not been investigated. Innovative Health Care Disparities Curriculum for Incoming Medical Students

PURPOSE: 1) To pilot a health disparities curriculum for incoming first year medical students and evaluate changes in knowledge. 2) To help students become aware of personal biases regarding racial and ethnic minorities. 3) To inspire students to commit to serving indigent populations.METHODS: First year students participated in a 5-day elective course held before orientation week. The course used the curricular goals that had been developed by the Society of General Internal Medicine Health Disparities Task Force. Thirty-two faculty members from multiple institutions and different disciplinary backgrounds taught the course. Teaching modalities included didactic lectures, small group discussions, off-site expeditions to local free clinics, community hospitals and clinics, and student-led poster session workshops. The course was evaluated by pre-post surveys.RESULTS: Sixty-four students (60% of matriculating class) participated. Survey response rates were 97-100%. Students' factual knowledge (76 to 89%, p<.0009) about health disparities and abilities to address disparities issues improved after the course. This curriculum received the highest rating of any course at the medical school (overall mean 4.9, 1 = poor, 5 = excellent).CONCLUSIONS: This innovative course provided students an opportunity for learning and exploration of a comprehensive curriculum on health disparities at a critical formative time.

“…The results indicate that Area Health Education Programs may wish to maximize their partnerships with family medical and other primary care specialty clerkship programs in effort to increase the number of medical students interested in pursuing a practice serving medically underserved patients upon graduation. Given that AHECs often work in partnerships with their community to develop clinical training programs that allow medical students to develop the skills and competencies to serve the medically underserved, the positive association between participation in an AHEC-sponsored clerkship experience and the intent to serve medically underserved falls in line with the research by Ferguson et al (2009) and Ko et al (2005)that states medical educational experiences can have a positive impact on the intent to practice after graduation. Given that there are many variables that may arise between when a medical student reports their intent to serve medically underserved patients and their eventual practice location, more follow-up research is needed to determine if the students actually end up practicing in medically underserved communities.…”

Does an AHEC-Sponsored Clerkship Experience Strengthen Medical Students’ Intent to Provide Care for Medically Underserved Patients?

Background: The mission of Area Health Education Centers (AHECs) is to recruit and educate students to serve as practicing health care professionals in rural, primary care, and medically underserved communities. Purpose: We sought to determine if participation in an AHEC-sponsored family medicine clerkship experiences during medical school are significantly associated with a self-reported intent to practice primary care in a medically underserved environment upon graduation. Method: The study was a prospective cohort study comparing third-year family medicine students with the Indiana University School of Medicine who participated in either an AHEC-sponsored family medicine clerkship to those who completed their required family medicine clerkship outside of the AHEC setting. Following the 160-hour clinical clerkship, all students completed a mandatory, electronic survey and were asked to self-report their intent to the following question: "Which of the following statements best describes the impact of the family medicine clerkship on your intention to provide care to underserved patients when you complete residency training?" The question was integrated into a mandatory post-clerkship evaluation form required by the Indiana University School of Medicine, Department of Family Medicine. A Chi square test of independence as well as a multivariate logistic regression analysis was used to determine the independent association of AHEC clerkship participation and reported intent. Results: A total of 1,138 students completed the survey. There were not significant differences in age, gender, race, and ethnicity between students that completed an AHEC clerkship and those that did not. After adjusting for gender, race, and ethnicity, AHEC participants were significantly more likely to report an intention to practice primary care in a medically underserved setting upon graduation. Female students were found to be 1.2 to 3.4 times as likely to report increased intent compared to male students (95% CI 1.241 to 3.394). Discussion: Participation in an AHEC-supported clerkship was associated with a significant increase in self-reported intent to practice primary care in a medically underserved setting. Additional research is required to determine if participation and/or reported intent are predictive of practice selection after graduation.

Abstract: Training in the UCLA/Drew program was independently associated with intention to practice medicine in underserved communities, suggesting that a medical education program can have a positive effect on students' goals to practice in underserved areas.
