Impact of the new fast track kidney allocation scheme for declined kidneys in the United Kingdom

Abstract: The FTKAS has led to effective utilization of the declined kidneys with outcome comparable to kidneys allocated through the standard scheme. Non-participation based on outcome concerns is mostly subjective while logistical issues need to be addressed.

“…The sharply higher utilization of older and high‐KDRI kidney donors in the UK compared with the US is one of the most salient observations from this study, which echo key messages from a recent comparison of kidney utilization in the US and France . Potential explanations for these US–UK differences include the following: Kidney allocation policy differences ◦Donor–recipient age‐matching points (UK) ◦Fast track offering scheme (UK) ◦High‐KDPI consent requirement (US) Differential reliance on biopsies for ruling in and ruling out transplantable kidneys Broader (US) vs narrower (UK) geography Variations in transplant finances Differences in dialysis practices Transplant center regulatory oversight differences Cultural differences influencing patient treatment expectations …”

“…Future policy refinements aimed at facilitating more dual kidney transplants (36,37)-i.e. two marginal kidneys into the same recipient-and establishing a formal mechanism for OPOs to expedite placement may help avoid unnecessary organ discard (38)(39)(40). In addition, the OPTN is exploring changes to the current paradigm used for monitoring transplant centers' performance to address the long-standing concern about risk aversion to using marginal organs (41,42).…”

“…This new methodology is designed to help the network monitor system‐level equity, in particular since policy proposals targeting other goals may adversely affect equity. For example, policy ideas such as deceased donor chains and expedited placement are focused on the OPTN's top goal of increasing the number of transplants but could conceivably hinder equity. However, tradeoffs may be tolerable.…”
