El colgajo microvascularizado de peroné es el estándar de oro para la reconstrucción mandibular por su gran capacidad de remodelación ósea; se ha evidenciado que el segmento distal del peroné puede tomar la forma y estructura de un cóndilo. Se reporta el caso clínico de un paciente adulto de sexo masculino diagnosticado con mixoma odontogénico, quien fue intervenido quirúrgicamente a través de una hemimandibulectomía con resección total del cóndilo y una reconstrucción mandibular primaria mediante un colgajo libre microvascularizado de peroné. Después de 30 meses de seguimiento se observó una neoformación condilar y adaptación progresiva a la cavidad glenoidea a pesar de que el extremo distal del peroné se encontró originalmente alejado de la posición planificada de manera digital.Palabras clave: Reconstrucción mandibular, colgajos de tejido libre, cóndilo mandibular, informe de caso clínico.

Gaps between medical needs and health services accessibility are defined as unmet medical needs (UMN). Previous studies assessing UMNs during the coronavirus disease 2019 (COVID-19) pandemic did not account for factors such as the type of medical insurance or chronic disease. Therefore, this study investigated various factors, including medical insurance and chronic diseases, affecting self-reported UMN during the pandemic in Daejeon Metropolitan City, South Korea. This cross-sectional study was conducted using Daejeon Metropolitan City Health Surveys data from 2020. The weighted prevalence of UMN was estimated among participants (n = 1494) aged ≥18 years, while chi-squared and logistic models were used to assess the association between UMN and participant characteristics. The prevalence of UMN was 6.4% (n = 97). Female sex and low education levels were significantly associated with reporting UMN. Medical aid beneficiaries were less likely to report UMN than adults with National Health Programs coverage. Participants with chronic diseases, such as hypertension and arthritis, and those with depression, were more likely to report UMN than adults without these conditions. The findings highlight the need for targeted efforts to decrease UMN in the COVID-19 pandemic, especially for women and individuals with low education levels. The results also indicate that high-risk disease groups should be targeted with additional enabling services to support the management of chronic conditions.

Introduction: The rise in telehealth adoption due to the emergence of COVID-19 may have had implications for men who experience barriers to accessing traditional forms of healthcare. This study sought to explorehow a sample of older men interacted with telehealth during the pandemic. Method: Data sourced from a cross-sectional, population-based questionnaire (completed from October 2020 to March 2021) were used to analyze the characteristics of older men’s (a) use of telehealth services, and (b) perceptions of telehealth in comparison to in-person healthcare using Andersen’s Behavioral Model of Health Services Use. Results: Of the 731 participants (mean age = 69 years; SD = 9.6), 241 (32.9%) had used telehealth services during pandemic restrictions. Most of them who had used telehealth (63.1%; 152/241) thought it was “just as good” as in-person, 4.1% (10/241) believed it was “better,” and 25.7% (62/241) thought it was “worse.” Men with more chronic conditions were more likely to (a) have used telehealth (odds ratio [OR], 1.44 [95% CI, 1.21–1.71]) and (b) perceived telehealth as “better” or “just as good” as in-person healthcare (OR, 1.63 [95% CI, 1.17–2.29]). Men with clinically significant depressive symptoms were more likely to view telehealth as worse than in-person care (OR, 0.32 [95% CI, 0.12–0.88]). Conclusion: While telehealth is acceptable to the majority of middle-aged and older men who have used it during the pandemic, attitudes may vary according to their current health issues. Men with more chronic conditions are more likely to feel positive about telehealth, while those with clinically significant depression symptoms are more likely to view it negatively. Healthcare providers should consider men’s needs and preferences when offering telehealth services.
