Impact of symptoms experienced by varenicline users on tobacco treatment in a real world setting

Abstract: This paper examines reported symptoms, nonsmoking rates, and medication use among 1018 smokers using varenicline in a randomized trial comparing three forms of behavioral support for smoking cessation (phone, web, or phone + web). One month after beginning varenicline, 168 people (17%) had discontinued the medication. Most (53%) quit due to side-effects and other symptoms. The most common side-effect among all users was nausea (reported by 57% of users). At one month post medication initiation, those not takin… Show more

“…Concerns have been raised about the incidence and severity of adverse events, particularly with regard to bupropion and varenicline (Barrueco et al, 2005;Halperin et al, 2009). Nonetheless, early discontinuation (within the first week) attributed to side effects or to a legitimate medical reason was uncommon, reported by only 4% of the sample overall.…”

“…In part because they require a doctor consultation, these medications have really only been tested in the presence of structured support. For both, adverse events leading to discontinuation are commonly reported (Barrueco et al, 2005;Halperin et al, 2009), although side effects are typically mild (Cahill et al, 2008;Zwar & Richmond, 2003). In Australia and the United Kingdom, a second prescription is needed to receive the full course of treatment of both bupropion and varenicline, which may act as a barrier to adherence.…”

“…Telephone follow-up surveys were conducted by nonintervention study staff approximately 21 days, 12 weeks, and 6 months after the target quit date to collect information on quit attempts, smoking, and medication adherence. At 21 days and 12 weeks, participants were also asked if during the past month, they experienced any of a number of treatment-related symptoms including known medication side effects (e.g., nausea, vomiting, change in appetite) and nicotine abstinence effects (e.g., irritability, desire to smoke; Halperin et al, 2009;McClure et al, 2009). Severity of symptoms and side effects endorsed in the past month was rated by participants on a Likert scale from 1 (very mild) to 5 (very severe), with 0 on this scale signifying "not present.…”

“…Detailed study methods and results are presented elsewhere (Halperin et al, 2008;McClure et al, 2009;Swan et al, 2010). Briefly, the COMPASS study was a randomized trial comparing the effectiveness of three forms of behavioral intervention for smoking cessation (delivered by Web, phone, or an integrated phone-Web program) in conjunction with varenicline.…”
