Impact of routine intraoperative cholangiography during laparoscopic cholecystectomy on bile duct injury

Abstract: The routine use of IOC during LC in a high-volume teaching centre was associated with a low incidence of BDI, and facilitated detection and repair during the same surgical procedure with a good outcome.

“…We believe, like Alvarez et al, that a high frequency of IOC results in a high degree of intraoperative diagnosis. 21 De Reuver et al reported only 19% of injuries detected intraoperatively, and Fischer et al reported 30% of injuries detected intraoperatively with 100% failure (5/5) when repair was performed intraoperatively. 9,15 This study presents a different population, an unselected national cohort of bile duct injuries were minor injuries less than five mm dominate.…”

“…Several additional techniques are described to prevent IBDI during LC. Despite the plethora of publications and debates, there is still no consensus regarding the best setting and method, although most surgeons would agree that the CVS [18] and IOC [27] are among the most popular and effective [27,28]. Both of these tools can be used via either laparotomy or laparoscopy.…”

“…It does not eliminate BDI, but may facilitate the intraoperative detection of injury and repair such that superior outcomes are achieved and mortality rates are reduced [1]. When detected during IOC after administering contrast agent and performing radiological imaging, cystic ducts should be dissected and cannulated.…”
