Impact of Preexisting and Induced Humoral and Cellular Immune Responses in an Adenovirus-Based Gene Therapy Phase I Clinical Trial for Localized Mesothelioma

Abstract: Little is known about the immune responses induced by recombinant adenoviral (Ad) vectors in humans. The humoral and cellular immune responses were therefore analyzed in 21 patients with localized malignancy (mesothelioma), who received intrapleurally high doses of a replication-defective Ad5 vector carrying a suicide gene. Eight of 21 patients had pretreatment titers of neutralizing antibodies (NAb) to Ad at > or =1:100. Peripheral blood mononuclear cells (PBMCs) proliferated in response to adenoviral 5 struc… Show more

“…In this respect, a recent study has demonstrated that pre-existing humoral immune responses did not preclude gene transfer. 30 Our strategy to date in human trials is to include all patients in trials, irrespective of in vitro measures of pre-existing immunity, and to develop correlations between the in vitro assays and efficiency of in vivo gene therapy.…”

“…The presence of vector-specific neutralizing antibodies inhibits vector readministration, especially when administered systemically, for example, in liver or lungs. 72 Interestingly, the blocking effects of neutralizing antibodies for vector readministration have less influence in the muscle, eyes or brain, primarily because of the physical barriers of lymphoid fluids in these organs. 73,74 Studies in rhesus monkeys have demonstrated that AAV and adenovirus vector elicit markedly different immunoglobulin isotypes.…”

“…A major concern for the future clinical application of Ad vectors is the possibility that preexisting immunity to an adenovirus of the same or cross-reactive serotype may strongly limit vector efficacy. This holds particularly true for adenovirus type 5, because of a high seroprevalence in the adult human population (21,22). We therefore determined how a preexisting anti-adenovirus type 5 immunity can affect i.m.…”
