Impact of Pneumococcal Vaccination on Pneumonia Hospitalizations and Related Costs in Ontario: A Population-Based Ecological Study

Abstract: Our results suggest that the publicly funded PCV immunization program is responsible for substantial reductions in pneumonia hospitalizations and related healthcare costs, among both young children eligible for publicly funded vaccination and other age groups not included in the publicly funded program.

“…Reductions in AC‐CAP hospitalizations of 36%, 20% and 16% in children <2, 2–4 and 5–17 years of age, respectively, and even more pronounced decreases in PL‐CAP, are consistent with reports from most other countries . These findings are concordant with previous publications from Sweden that reported up to 37% reduction of inpatient all‐cause pneumonia in children < 2 years in 2012 .…”

“…Reports on the indirect effect of PCV‐CIP on CAP in the elderly have been inconsistent. The first reports on reduced incidence of AC‐CAP hospitalizations came from the United States, and thereafter from Canada and Finland . However, in a recent study that adjusted for secular trends using the synthetic control method, there was no observed reduction of hospitalization for all‐cause pneumonia after PCV‐CIP introduction in any of five countries in the Americas (the United States, Mexico, Ecuador, Brazil and Chile) .…”

“…The estimated net savings may be underestimated, as our assumptions were generally conservative. For example, Luca and colleagues found that incidence of pneumonia hospitalizations and costs have declined by 35-45% in the PCV era compared with pre-PCV-era data [5]. Our findings suggested a 10% reduction in pneumococcal pneumonia, substantially lower than that reported in the Luca study.…”

“…Since their inclusion in routine IIPs, PCVs have substantially reduced the incidence of IPD, pneumonia, and AOM in Canada, which translated into a reduction in health care resource use [3][4][5][6][7]. Public health budgets are constrained in Canada, with limited funds available for the introduction of new vaccine programs.…”
