Dengue and Zika viruses cocirculate annually in endemic areas of Mexico, causing outbreaks of different magnitude and severity every year, suggesting a continuous selection of Flavivirus variants with variable phenotypes of transmissibility and virulence. To evaluate if Flavivirus variants with different phenotypes cocirculate during outbreaks, we isolated dengue and Zika viruses from blood samples of febrile patients from Oaxaca City during the 2016 and 2019 epidemic years. We compared their replication kinetics in human cells, susceptibility to type I interferon antiviral response, and the accumulation of subgenomic RNA on infected cells. We observed correlations between type I interferon susceptibility and subgenomic RNA accumulation, with high hematocrit percentage and thrombocytopenia. Our results suggest that Flaviviruses that cocirculate in Oaxaca, Mexico, have variable sensitivity to the antiviral activity of type I interferons, and this phenotypic trait correlates with the severity of the disease.

Objective The uncertainty of dengue's progression from infection to its severe form represents a major health care challenge, especially in children. Clinical identification of impending clinical manifestations of severe dengue (SD), along with proper and immediate management, is crucial. Thus, this study assesses the ability of warning signs to predict SD infection in pediatric patients. Methods This cross-sectional study utilized purposive sampling using medical records from January 2015 to December 2020. Children aged 0 to 18 years diagnosed with dengue fever and SD according to the World Health Organization's 2009 criteria were included. Discussion Multivariate analysis revealed that abdominal pain (odds ratio [OR]: 16.34; 95% confidence interval [CI]: 3.78–70.64; p < 0.001), fluid accumulation (OR: 10.51, 95% CI: 1.17–94.3; p = 0.036), mucosal bleeding (OR: 4.77; 95% CI: 1.27–17.91; p = 0.021), lethargy (OR: 94.37; 95% CI: 4.92–180.79; p = 0.003), hepatomegaly (OR: 17.57; 95% CI: 2.14–144.13; p = 0.008), and increased hematocrit concurrent with a rapid decrease in platelets (OR: 6.89; 95% CI: 1.79–26.51, p = 0.005) were associated with SD infection, with a high quality of discrimination (area under the curve [AUC] = 0.96) and a high quality of fit (p = 0.73). Receiver operating characteristic analysis demonstrated that 1.5 warning signs was the optimal cut-off for predicting SD infection, with a sensitivity of 90.9 and a specificity of 89.8%. Conclusion All six warning signs were significantly associated with SD infection. The optimal cut-off for predicting SD was 1.5 warning signs.

Dengue infection is a viral disease that is transmitted through mosquitoes, especially the Aedes aegypti species infected by dengue virus. Nutritional status is thought to be one of the factors that can affect the severity of dengue infection. This study aimed to determine the relationship between nutritional status and the severity of dengue infection in pediatric patients at Prof. Dr. R. D. Kandou Hospital Manado. This was an observational and analytical study with a case control design using secondary data of the medical record of Pediatric Department at Prof. Dr. R. D. Kandou Hospital. Samples were taken using the purposive sampling technique. The results obtained a total sample of 332 samples that matched the inclusion criteria. Data were analyzed using the Chi-Square test with Jamovi 1.6.23 for Windows program resulting in a p-value of 0.205 (p>0.05) for the relationship between nutritional status and the severity of dengue infection in the pediatric patients. In conclusion, there is no relationship between nutritional status and the severity of dengue infection in pediatric patients at Prof. Dr. R. D. Kandou Hospital, Manado.Keywords: dengue infection; nutritional status; children Abstrak: Infeksi dengue merupakan penyakit virus yang penularannya melalui perantara gigitan nyamuk terutama spesies Aedes aegypti yang terinfeksi oleh virus dengue. Jenis penelitian ialah analitik observasional dengan rancangan case control menggunakan data sekunder dari Bagian Ilmu Kesehatan Anak RSUP Prof. Dr. R. D. Kandou Manado. Sampel diambil menggunakan teknik purposive sampling, Hasil penelitian mendapatkan total sampel sebanyak 332 sampel yang sesuai dengan kriteria inklusi. Data penelitian dianalisis menggunakan uji Chi-Square dengan program Jamovi 1.6.23 for Windows yang mendapatkan nilai p=0,205 (p>0,05) untuk hubungan antara status nutrisi dengan tingkat keparahan infeksi dengue pada pasien anak di RSUP Prof. Dr. R.
