Impact of neoadjuvant chemoradiation on pathologic response and survival of patients with locally advanced rectal cancer

Abstract: CT combined with RT has increased tumoral response and survival rate. Nodal downstaging and pCR were higher in the GROUP 2. The 5-year OS and DFS were significantly superior in CT + RT arms. Patients with pathologic response showed a better DFS. Adjuvant CT had no impact on LRC, DFS nor on OS.

“…However, ‘overall survival’ was defined in only 20 studies with six different definitions . ‘Disease‐free survival’ was reported in 43 (22.2%) articles and defined in 27 with 16 unique definitions . The most commonly used measures of short‐term operative mortality were ‘30‐day mortality’ and ‘postoperative mortality’, which were reported in 16 (8.2%) and 14 (7.2%) of the included publications, respectively.…”

“…A further 13 (19.7%) individual outcomes concerned the time to recurrence, six (9.1%) concerned disease progression and two (3.0%) reported second primary malignancies. The two most frequently reported cancer‐related individual outcomes were ‘local recurrence’ (recorded in 44 studies and defined in 19, in 15 different ways) and ‘distant recurrence’ (recorded in 23 studies and defined in four, in four different ways) .…”

“…When considering treatment options for rectal cancer, preoperative evaluations are important, because decisions regarding surgery and preoperative concurrent chemoradiotherapy are dependent on tumour location, mesorectal fascia involvement, and nodal status 6,7,9–13. Rectal MRI is commonly performed preoperatively for evaluating the mesorectal fascia or adjacent organ involvement, and nodal staging 6–9.…”

“…When considering only those patients cT3-4 who had downstaging to ypT0-2, we found a significantly better locoregional control (100% vs 89, p = 0.027), PFS (88% vs 43%, p = 0.003) and OS (89% vs 77%, p = 0.048). Adjuvant chemotherapy had no impact on locoregional control, PFS or on OS (Conde, 2010).…”
