Introduction and aims. Pregnancy is a physiological state for a woman. However, if pathology develops, the course of pregnancy is affected by both mental and biological stress. Stress experienced during pregnancy may irreversibly alter the cerebral structures in the foetus. Moreover, chronic stress is likely to cause miscarriage, preterm delivery, IUGR, low birth weight, etc. The aim of the study was to analyse the stress level in hospitalized pregnant women with account forsociodemographic factors. Material and methods. The study was carried out in a group of 140 pregnant women hospitalized in the Department of Pregnancy Pathology, Independent State Clinical Hospital No.1 in Lublin. Inclusion criteria for the study were: age 18 or older, established pregnancy, and hospitalization for more than 2 weeks in a pregnancy pathology department. The participants were diagnostically surveyed by the Perceptible Stress Scale (PSS-10) and a questionnaire created for the study. Results. Almost every second (76; 54.3%) pregnant woman demonstrated low level of stress, 13.6% (n=19) respondents showed average level, and high level was detected in 32.1% (n= 45) of the respondents. A statistically significant correlation was found between the level of perceived stress and education (p=0.031), financial situation (p=0.017). Conclusions. During hospitalization, nearly every third pregnant woman was affected by a high level of stress. The very fact that the health of the child and/or pregnant mother is found to be at risk is already a source of stress for the woman. Higher intensity of stress - statistically significant concerned mothers with professional education and average financial situation. Data from reports in the literature indicate that stress during pregnancy can also negatively affect the health of the psycho-physical diady- mother child. Further research in this area is needed, along with the possibility of taking measures to promote the psychophysical comfort of hospitalized mothers.

Fetal heart rate (FHR) monitoring is one of the central parts of obstetric care. Ultrasound-based technologies such as cardiotocography (CTG) remain the most common method for FHR monitoring. The CTG’s limitations, including subjective interpretation, high interobserver variability, and the need for skilled professionals, led to the development of computerized CTG (cCTG). While cCTG demonstrated advantages, its superiority over visual interpretation remains inconclusive. This has prompted the exploration of alternatives like noninvasive fetal electrocardiography (NIFECG). This review explores the landscape of antenatal FHR monitoring and the need for remote FHR monitoring in a patient-centered care model. Additionally, FHR monitoring needs to evolve from the traditional approach to incorporate artificial intelligence and machine learning. The review underscores the importance of aligning fetal monitoring with modern healthcare, leveraging artificial intelligence algorithms for accurate assessments, and enhancing patient engagement. The physiology of FHR variability (FHRV) is explained emphasizing its significance in assessing fetal well-being. Other measures of FHRV and their relevance are described. It delves into the promising realm of NIFECG, detailing its history and recent technological advancements. The potential advantages of NIFECG are objective FHR assessment, beat-to-beat variability, patient comfort, remote prolonged use, and less signal loss with increased maternal body mass index. Despite its promise, challenges such as signal loss must be addressed. The clinical application of NIFECG, its correlation with cCTG measures, and ongoing technological advancements are discussed. In conclusion, this review explores the evolution of antenatal FHR monitoring, emphasizing the potential of NIFECG in providing reliable, home-based monitoring solutions. Future research directions are outlined, urging longitudinal studies and evidence generation to establish NIFECG’s role in enhancing fetal well-being assessments during pregnancy.

The foetal period constitutes a critical stage in the construction and organisation of the mammalian nervous system. In recent work, we have proposed that foetal brain development is supported by bottom-up (interoceptive) inputs from spontaneous physiological rhythms such as the heartbeat (Corcoran et al., in press). Here, we expand this visceral afferent training hypothesis to incorporate the development of top-down (allostatic) control over bodily states. We propose that the brain’s capacity to actively modify and regulate the afferent feedback it receives through interoceptive (and other sensory) channels is crucial for establishing basic forms of agentic control and grounding the distinction between self and other. We further suggest that individual differences in the way these training regimes are implemented (or disrupted) might impact developmental trajectories in gestation and infancy, including the potentiation of neurobehavioural impairment and disease risk in later life. This analysis raises the important question of whether the deleterious effects of prenatal adversity (e.g., premature birth) can be mitigated via targeted visceral afferent training interventions.
