Impact of hormonal therapy and other adjuvant therapies on contralateral breast volume change after implant-based breast reconstruction

Abstract: BackgroundAdjuvant therapy after breast surgery, including tamoxifen or aromatase inhibitors, improves the postoperative outcomes and long-term survival of breast cancer patients. The aim of this study was to determine whether volume changes occurred in the contralateral breast during hormonal or other adjuvant therapies.MethodsThis study reviewed 90 patients who underwent unilateral breast reconstruction between September 2012 and April 2018 using tissue expanders and a permanent implant after the surgical re… Show more

“…Skin-sparing mastectomy (SSM) or nipple-sparing mastectomy (NSM) were performed based on oncological considerations. Before surgery, shape and volumetric analysis of the breast, perioperative conditions such as radiotherapy, and patient preferences were considered to determine which type of reconstruction should be used, autologous or implant reconstruction 5 . In cases of reconstruction using an implant, skin flap condition was judged by considering clinical conditions such as mastectomy flap thickness, color, and indocyanine green angiography, which was performed immediately after mastectomy.…”

If the circulation of the skin flap after surgery was considered favorable and the nipple-areolar complex (NAC) was preserved, prepectoral DTI was conducted.…”

“…These findings might occur from a decrease in volume of the contralateral healthy breast through a reduction of the breast parenchyma in patients who received adjuvant hormone therapy. 28 The reconstructed breast volume ratio increased over time in the case of the delayed reconstruction. The volume ratio of the reconstructed breast significant increased by 0.4 % (coefficient = 0.004) over one month after surgery (p = 0.043).…”

“…2 In Western medicine, the treatments available for breast cancer are surgery, such as reconstructive surgery, mammoplasty, radical mastectomy, etc; after surgery, if necessary, radiotherapy treatments; and drugs such as estrogen modulator, chemotherapy, hormonal chemotherapy, etc. 3 This article was written following Hippocrates thoughts that said "it is important to consider other ancient medical traditions prior to the knowledge we have nowadays" and for this reason, the author will combine traditional Chinese medicine reasoning with what we have nowadays regarding Western medicine´s point of view to show different perspectives regarding breast cancer genesis. 4…”
