Impact of Glucocorticoids on Cardiovascular System—The Yin Yang Effect

Abstract: Glucocorticoids are not only endogenous hormones but are also administered exogenously as an anti-inflammatory and immunosuppressant for their long-term beneficial and lifesaving effects. Because of their potent anti-inflammatory property and ability to curb the cytokines, they are administered as lifesaving steroids. This property is not only made use of in the cardiovascular system but also in other major organ systems and networks. There is a fine line between their use as a protective anti-inflammatory and… Show more

“… 61 Additionally, the genetic marker could also predispose an individual to the risk of atherosclerosis especially genes that are related to the immune response such as TNFSF13B . 62 , 63 The TNFSF13B gene encodes the BAFF protein, which plays a critical role in B-cell modulation. The increase in serum BAFF levels is higher in individuals at risk of atherosclerosis compared to those who are not at risk.…”

“…On the other side, one must not omit the fact that some of our chronic patients' comorbidities and elevated GLR may be involved in a cause-effect relationship with the specific anti-myasthenic treatments. Specifically, mainly the chronic administration of corticosteroids is a probable factor as it is a known fact that doses higher than 7.5 mg/day of prednisone or equivalent doses of prednisone are associated with cardiovascular risks and, to a lesser extent, mood disorders [31,42]. Moreover, corticosteroids have been acknowledged to induce metabolic and hematological side effects such as an increase in plasma glucose and circulating polymorphonuclear levels, while the levels of the other granulocytes, lymphocytes and monocytes, decrease mainly by the redistribution of these cells [43].…”

“…The enzyme is also expressed in endothelial cells, where it causes vascular hypertrophy and fibrosis. [ 35 ] In a cohort study, Wei et al . discovered that patients receiving high-dose exogenous steroids equivalent to prednisolone ≥ 7.5 mg/d had a 2.5-fold increased risk for cardiovascular events compared to those who received low-dose steroids.…”
