Impact of COVID-19 outbreak on ischemic stroke admissions and in-hospital mortality in North-West Spain

Abstract: Background and purpose Spain has been one of the countries heavily stricken by COVID-19. But this epidemic has not affected all regions equally. We analyzed the impact of the COVID-19 pandemic on hospital stroke admissions and in-hospital mortality in tertiary referral hospitals from North-West Spain. Methods Spanish multicenter retrospective observational study based on data from tertiary hospitals of the NORDICTUS network. We recorded the number of patients admitted for ischemic stroke between 30 December 20… Show more

“…We performed a retrospective study using our institutional stroke registry to investigate pre- and intra-hospital delays during the COVID period. We then compared our institutional data to the Get with the Guidelines (GWTG) database to analyze our experience by comparing regional and national trends in stroke volume [ 7 , 8 ].…”

“…Interestingly, despite a reduction in the number of hospital admissions for AIS, a higher proportion of patients received reperfusion therapies compared to both pre-COVID-19 era in 2020 and from the same period in 2019. However, previous reports have shown a decreased number [21] , [22] , [23] but a relatively unchanged proportion 6 , 18 of patients who received reperfusion therapy during COVID-19 pandemic. The first confirmed case in Virginia was reported on March 7, 2020, and the first confirmed case was admitted to our facility on March 12, 2020.…”

“…The impact of the COVID-19 pandemic on hospital admissions for vascular diseases, including acute coronary syndrome [14] , [15] and acute ischemic stroke, 6 , [15] , [16] , [17] , [18] , [19] , [20] has been reported. A 36.4% reduction in AIS admissions was seen in Brazil after COVID-19 compared to the same period in 2019.…”

“…On the contrary, health authorities emphasised that suspected stroke victims should continue to seek medical attention and call the emergency medical services despite the lockdown. There are contradicting results regarding prehospital delay and the overall number and proportion of ischemic stroke patients receiving intravenous thrombolysis during the lockdown [ 1 , 2 , 5 , 7 , 8 , 10 13 ] and reduced standard of care and delayed stroke care pathways with increased door-to-needle time have also been reported [ 1 , 2 , 5 , 7 , 8 , 10 13 ]. The proportion receiving thrombolysis if they reached our hospital within the time window was similar for all three periods, indicating that the hospital’s acute stroke workflow and pathway were not affected to a degree harming patients [ 10 , 20 ].…”
