AimThe Coronavirus Disease 2019 (COVID‐19) pandemic affected the allocation of various medical resources to several areas, including intensive care units (ICUs). However, currently, its impact on the short‐term postoperative outcomes of gastrointestinal cancer surgeries remains unclear. We aimed to evaluate the impact of the pandemic on the incidence of complications occurring after low anterior resection in patients with rectal cancer in Japan.MethodsData from the Japanese National Clinical Database between 2018 and 2021 were retrospectively examined. The primary outcome of the study was the postoperative morbidity and mortality rates before and after COVID‐19 pandemic. Moreover, the postoperative ICU admission rate was assessed. Morbidity and mortality rates were also assessed using a standardized morbidity/mortality ratio (SMR, the ratio of the actual number of incidences to the expected number of incidences calculated by the risk calculator).ResultsThis study included 74 181 patients, including 43 663 (58.9%) from COVID‐19 epidemic areas. The mean actual incidences of anastomotic leakage (AL) and pneumonia during the study period were 9.2% and 0.9%, respectively. The SMRs of these complications did not increase during the pandemic but those of AL declined gradually. The mean 30‐day mortality and operative mortality rates were 0.3% and 0.5%, respectively. Moreover, SMRs did not change significantly in the pandemic or regional epidemic status. The ICU admission rate temporarily decreased, especially in the epidemic areas.ConclusionAlthough the pandemic temporarily decreased the ICU admission rate, its impact on short‐term outcomes following low anterior resection in patients with rectal cancer was insignificant in Japan.

Background: The influence of the coronavirus disease (COVID-19) pandemic on the number of newly diagnosed gynecological cancers has not been extensively investigated in Japan. This study aimed to determine the impact of COVID-19 on the incidence of gynecological cancer. Methods: Using the registry database of the Japanese Society of Obstetricians and Gynecologic Oncology, the distribution of the number of patients by clinical staging or tumor-node-metastasis (TNM) classifications before and during the COVID-19 pandemic was analyzed to compare the trends. The clinical staging classification of cervical cancer in Japan was based on the International Federation of Gynecology and Obstetrics (FIGO)-2008 from 2018 to 2020 and on the FIGO-2018 from 2021. Since FIGO-2018 classified N1 cases as stage IIIC, we focused on T classification without reference to the clinical staging (FIGO staging) of patients with cervical cancer in 2021. Results: The number of patients with endometrial cancer and malignant ovarian tumor of all clinical stages increased uniformly year-by-year while the number of patients with stage III cervical cancer increased sharply in 2021 owing to the adoption of the revised classification. On comparing cases with cervical cancer in 2020 and 2021, we found that T1 cases decreased and T2 and T3 cases increased in 2021 compared to those in 2020 (p=0.006). CIN3/AIS decreased in 2020 compared to in 2019 but increased again in 2021. The number of patients with cervical cancer decreased in most prefectures in 2020. Conclusion: The incidence of locally advanced cervical cancer increased during the COVID-19 pandemic.

Hintergrund Die Pandemie durch das Sars-CoV2- Virus (COVID-19) hat die (medizinische) Welt verändert und in zahlreichen Facetten auch Einfluss auf die Gastroenterologie genommen. Ziel der Arbeit Seit Beginn der Pandemie wurde eine große Menge an Informationen über das „severe acute respiratory syndrome coronavirus type 2“ (SARS-CoV-2) und COVID-19 publiziert. Diese Arbeit soll einen Einblick in die Aspekte geben, die für die Gastroenterologie von besonders hoher Relevanz sind. Material und Methoden Es werden Grundlagenarbeiten, Fallserien und Expertenempfehlungen zu gastroenterologischen Aspekten der COVID-19-Pandemie diskutiert. Ergebnisse und Diskussion Gastroenterologische Symptome treten bei COVID-19 zwar häufig auf, haben aber keinen negativen Einfluss auf den Krankheitsverlauf. Ob eine fäkal-orale Übertragung von COVID-19, wie sie durch den Nachweis viraler RNA im Stuhl naheliegt, möglich ist, ist noch nicht abschließend geklärt. Die Fortführung einer immunsuppressiven Therapie bei Patienten mit chronisch-entzündlichen Darmerkrankungen während der COVID-19-Pandemie ist sicher. Eine systemische Steroidtherapie birgt jedoch ein höheres Risiko für einen schweren Verlauf der Infektionskrankheit. Eine Impfung gegen COVID-19 ist auch bei Patienten unter immunsuppressiver Therapie sicher und effektiv. Die COVID-Pandemie hat sektorübergreifend signifikante Auswirkungen auf Patienten- und Versorgungsstrukturen, nicht nur in der Gastroenterologie. Neben den Herausforderungen für die Aufrechterhaltung einer guten medizinischen Versorgung aller Patienten liegen zukünftige Herausforderungen auch darin, die Tätigkeit im Gesundheitssystem durch strukturelle, inhaltliche und psychologische Unterstützung der Akteure attraktiv zu halten und auch unter den Umständen einer Pandemie eine qualitativ hochwertige Ausbildung im pflegerischen und ärztlichen Bereich sicherzustellen.
