INTRODUCTIONHearing loss is prevalent in one in three people over the age of 50, and two-thirds of those older than 70 years. 1 Hearing loss in older adults impairs verbal communication, contributes to social isolation, 2 and has been independently associated with poorer cognitive functioning [3][4][5] and incident dementia. [6][7] While hearing aids improve audibility for most individuals with mild to moderate hearing loss, those who suffer from severe to profound hearing loss generally may acquire greater benefit from electrical hearing provided by a cochlear implant (CI). 8 Currently, the number of older adults in the United States who would potentially meet audiologic criteria for a CI is approximately 150,000, 1 but less than 5% of these older adults are likely to have received a CI System. 9 Many factors could potentially account for the low rate of CI use in older adults, such as a general perception of hearing loss being an inconsequential part of aging and poor awareness of and access to cochlear implantation. Additionally, there are concerns about whether older adults could practically benefit from CI on a daily basis. Older adults can consistently acquire improved speech perception scores after CI in the clinical setting, 10 but patterns of CI use by older adults has received little review. Previous studies have reported results only in small cohorts of older adults with limited durations of follow-up. [11][12][13][14][15][16][17][18] The purpose of our study was to investigate long-term rates of CI use in a consecutive case series of older adults (≥60 years) who received their first CI from 1999-2011. MATERIALS AND METHODS STUDY COHORTWe queried the Johns Hopkins Listening Center database to retrieve all patients ≥ 60 years who received a first CI from 1999 to June 2011 (n = 447). The purpose of this study was to investigate rates of CI use in older adults which is generally defined by the United Nations as adults ≥ 60 years. 33 Of these patients, we were able to successfully obtain follow-up information from the patient or a proxy respondent for 397 individuals (89%) from June to August 2012, and these individuals comprise our analytic cohort. Characteristics of "responders" (n = 397) and "non-responders" (n = 50) did not differ significantly by age of implantation, onset of hearing loss, side of implantation, or manufacturer (data not shown). Non-responders were more likely to have been implanted earlier in the study time period than responders (p < .001). Of the 50 non-responders, 27 (54%) had died based on Social Security Death Index records. For these deceased non-responders, we were unable to obtain proxy information on their CI use at the time of death due to unavailable contact information for surviving family members. Purpose: Cross-cultural adaptation and translation of the Nijmegen Cochlear Implant Questionnaire (NCIQ) into Brazilian Portuguese and analysis of quality of life (QoL) results in adults with cochlear implant (CI). Methods: The NCIQ instrument was translated into Brazilian Portuguese and culturally adapted. After that, a cross-sectional and clinical QoL evaluation was conducted with a group of 24 adults with CI. Results: The questionnaire title in Brazilian Portuguese is 'Questionário Nijmegen de Implantes Cocleares' (NCIQ-P). The version of the NCIQ questionnaire translated into Brazilian Portuguese presented good internal consistency (0.78). The social and physical domains presented the highest scores, with the basic and advanced sound perception subdomains achieving the highest scores. No correlation between gender and time of device use was found for the questionnaire domains and subdomains. Conclusion: The cross-cultural adaptation and translation of the NCIQ into Brazilian Portuguese suggests that this instrument is reliable and useful for clinical and research purposes in Brazilian adults with CI. RESUMOObjetivo: Traduzir e adaptar culturalmente para o Português Brasileiro o Nijmegen Cochlear Implantation Questionnaire (NCIQ) e descrever os resultados de qualidade de vida em adultos. Método: Tradução e adaptação cultural do NCIQ. Depois desta etapa, foi realizado um estudo transversal e clínico de avaliação da qualidade de vida em um grupo de 24 adultos usuários de implante coclear (IC). Resultados: O título do questionário na versão traduzida para o Português Brasileiro foi definido em Questionário Nijmegen de Implantes Cocleares (NCIQ-P). A versão traduzida do questionário NCIQ-P apresentou boa consistência interna para todos os domínios existentes no questionário (0,78). Os resultados de qualidade de vida em adultos demonstraram que os domínios mais pontuados foram o social e o físico, sendo os subdomínios percepção básica e avançada os de maior pontuação. Não foi observada correlação entre gênero e tempo de uso do dispositivo para os domínios e subdomínios existentes no questionário. Conclusão: A tradução e adaptação cultural do NCIQ-P sugere que o instrumento é válido e útil para o uso clínico e de pesquisa em adultos usuários de IC e falantes do Português Brasileiro.Santos et al. Current Trends in Treating Hearing Loss in Elderly People: A Review of the Technology and Treatment Options – A Mini-Review

Background: According to the World Health Organization (WHO), by 2025 there will be approximately 1.2 billion people in the world over the age of 60, which marks a shift in world population to a greater proportion of older people. An estimated 70–80% of adults between 65 and 75 years of age suffer from presbycusis, or age-related, bilateral sensorineural hearing loss (HL) in the high frequencies. Presbycusis is correlated with decreased quality of life (QoL) and depression and according to WHO, is a leading cause of years lived with disability in the adult years. Objective: The purpose of the current study was to review the body of literature on treatment options and considerations for the elderly population, as there is a variety of audio-technology available today to treat presbycusis. Methods: A PubMed literature search was conducted using the keywords ‘presbycusis/presbyacusis/geriatric AND hearing aids/cochlear implants/electric acoustic stimulation/middle ear implants’ and ‘elderly AND cochlear implants’. References were also mined from papers found. Results: 431 articles were considered in this review of treatment options for elderly patients suffering from presbycusis. Conclusion: Hearing aids and cochlear implants (CIs) are the most commonly used devices for treating mild-severe presbycusis. Reported outcomes with hearing aids indicate they are an effective method for treating mild-moderate HL in cases where the patient is appropriately fitted and is willing, motivated, and able to use the device. Depending on the type and severity of the HL and the specific needs of the patient, electric-acoustic stimulation and active middle ear implants may also be appropriate solutions for treating presbycusis. Finally, very positive QoL and speech perception outcomes have been documented in treating severe-profound presbycusis with CIs. In some studies, QoL outcomes have even exceeded expectations of elderly patients.
