Impact of and Interaction between the Availability of Prior Examinations and DBT on the Interpretation of Negative and Benign Mammograms

Abstract: Rationale and Objective To assess the interaction between the availability of prior examinations and digital breast tomosynthesis (DBT) in decisions to recall a woman during interpretation of mammograms. Materials and Methods Eight radiologists independently interpreted twice 36 mammography examinations, each of which had current and prior full field digital mammography images (FFDM) and DBT under a HIPPA compliant IRB approved protocol (written consent waived). During the first reading three sequential rati… Show more

“…The possibility was as high as 81%-97% among the patients with a burr on edge of tumor, a positive burr sign hinting possible breast cancer to a great extent (Smith, 2005;Gong et al, 2006) Poplack et al (Poplaek et al, 2007) deemed that DBT images, especially the images of tumors, were superior to traditional FFDM images. From the results, we might make judgment of a benign tumor for a mass with a smooth edge, clear borders and a fine uniform bright ring around it, which is consistent with the previous study (Hakim et al, 2014). This might provide a more accurate basis of images for the minimally invasive surgery expected by the women who love beauty.…”

“…Current studies by Hakim et al. also reported that recall rates were able to be reduced with the addition of prior images by 34% and 32% without and with DBT images (16,17) although their study did not describe the effect of prior mammograms on diagnostic performance in terms of sensitivity and specificity.…”

“…The use of prior mammograms in the reading of current mammograms had previously been reported to increase specificity especially in the screening setting (13,15), while also increasing the detection of true-positive findings in the diagnostic setting (1). Current studies by Hakim et al also reported that recall rates were able to be reduced with the addition of prior images by 34% and 32% without and with DBT images (16,17) although their study did not describe the effect of prior mammograms on diagnostic performance in terms of sensitivity and specificity.…”

“…Study Population FFDM and DBT images obtained in 153 women who ranged in age from 37 to 83 years (mean age, 53.7 years 6 9.3 [standard deviation]) were sequentially selected for this study from a pool of 519 studies (predominantly from prior research studies), 36 of which were used in a pilot study (16) and were excluded from this study. The selection of cases was based on the availability of a prior FFDM study obtained between 1 and 3 years prior to the current FFDM and DBT studies of interest and the requirement to fulfill a desired distribution of verified outcomes, breast densities, and abnormality types.…”
