Impact of Alveolar Recruitment Maneuver in the Postoperative Period of Videolaparoscopic Bariatric Surgery

Abstract: We conclude that ARM is a safe and effective technique when used for prevention of pulmonary complications in patients undergoing bariatric surgery, resulting in more favorable radiological and spirometric findings in the experimental group compared to the control group in the PO.

“…However, even with the proposed treatment showing a weak effect, the intubation time and consequently the MV time were higher in the control group. Although a significant effect of the treatments proposed in this study cannot be shown, the study by Remístico et al 13 showed shorter duration of surgery in the group receiving alveolar recruitment with 30 cm H 2 O PEEP.…”

“…9---11 Some ventilatory strategies have been used postoperatively in an attempt to improve gas exchange through the use of positive pressure maneuvers aiming at alveolar recruitment and even reducing the surgical time. 12,13 However, the literature on the use of positive pressure preoperatively as a prophylactic manner is still scarce.…”

“…During pneumoperitoneum pulmonary resistance increases. Alveolar recruitment by repeated lung inflation to 50 mmHg or with vital capacity maneuver followed mechanical ventilation with PEEP 10-12 cmH2O, and beach chair position has been effective at preventing lung atelectasis and is associated with better oxygenation in obese patients undergoing laparoscopic bariatric surgery (34)(35)(36)(37). In addition to pneumoperitoneum, body mass is an important determinant of respiratory function during anaesthesia in obese patients.…”
