Impact of a care pathway for COPD on adherence to guidelines and hospital readmission: a cluster randomized trial

Abstract: PurposeCurrent in-hospital management of exacerbations of COPD is suboptimal, and patient outcomes are poor. The primary aim of this study was to evaluate whether implementation of a care pathway (CP) for COPD improves the 6 months readmission rate. Secondary outcomes were the 30 days readmission rate, mortality, length of stay and adherence to guidelines.Patients and methodsAn international cluster randomized controlled trial was performed in Belgium, Italy and Portugal. General hospitals were randomly assign… Show more

“…Affecting approximately 250 million people worldwide [1], Chronic Obstructive Pulmonary Disease (COPD) is a common, preventable and treatable disease characterized by progressive airway obstruction, deterioration in lung function and increased mortality. As a leading cause of hospital admissions, COPD is one of the most significant public health concerns [1][2][3]. With the progression of the disease, acute exacerbations of COPD increase, resulting in an intensive healthcare utilization with frequent physician visits, recurrent emergency department (ED) visits and hospitalizations, and a deterioration of the patient's health condition and quality of life [3][4][5].…”

“…The median number of hospitalization days we found compares favorably not only to control regions in the Netherlands but also to 2016 data from Belgium, Italy, and Portugal in a cluster-randomized trial. 27 Actually, the median number of days per hospitalization was 12 in their study and five in ours. An important difference in design was the fact that the latter study was hospital-based only, as opposed to ours.…”

“… 20 Care pathways for in-hospital management of exacerbated COPD exist, with positive effects on 30-day readmission rate. 21 The present study shows that a systematic approach seems to be beneficial for all patients with asthma as well as COPD referred to secondary care pulmonology. However, we based our conclusions on a limited number of patients who were evaluated with the systematic approach.…”
