ObjectiveTo determine the association between occupational factors, particularly psychosocial factors, and hypertension.DesignDescriptive and analytical cross-sectional study using logistic multivariate regression.SettingFifteen cotton ginning plants in Benin.ParticipantsPermanent and occasional workers in the cotton ginning industry.Data collectionData on sociodemographic, occupational, behavioural and clinical history characteristics were collected using a number of standardised, interviewer-administered questionnaires. These questionnaires were based on the WHO’s non-communicable disease questionnaire, Karasek questionnaire and Siegrist questionnaire. Weight, height and blood pressure were measured. Any worker with systolic blood pressure ≥140 mm Hg and/or diastolic blood pressure ≥90 mm Hg according to the WHO criteria was considered hypertensive, as was any subject on antihypertensive treatment even if blood pressure was normal.ResultsA total of 1883 workers were included, with a male to female ratio of 9.08. Of these, 510 suffered from hypertension (27.1%, 95% CI 25.1 to 29.2). In the multivariate analysis, the risk factors identified were occupational stress (adjusted OR (aOR)=3.96, 95% CI 1.28 to 12.2), age ≥25 years (aOR=2.77, 95% CI 1.55 to 4.96), body mass index of 25–30 kg/m2(aOR=1.71, 95% CI 1.32 to 2.2), body mass index >30 kg/m2(aOR=2.74, 95% CI 1.84 to 4.09), permanent worker status (aOR=1.66, 95% CI 1.44 to 2.41) and seniority in the textile sector >5 years (aOR=2.18, 95% CI 1.7 to 2.8). Recognition at work emerged as an effect-modifying factor subject to stratification.ConclusionsOccupational factors, particularly job strain and recognition at work, are modifiable factors associated with hypertension in the ginning plants sector and deserve to be corrected through occupational health promotion and prevention.

Stress can be described as the perception of discomforts physically, psychologically, or physico-psychologically. During stress, the perceived discomfort indicates there is a deviation from homeostasis. In stress, there is a nonspecific physiological response to stressors, a group of stress-inducing phenomena. Stress-inducing phenomena can be defined as environmental insults, such as perturbed levels of light, temperature, chemicals, ambient oxygen, and noise. Response to stress occurs via the chemical messenger-mediated sympathetic nervous system including the autonomic-adrenal axis. Furthermore, the chemical messenger-mediated sympathetic nervous system determines nonhormonal effects which are often devised as general stress markers. Examples of general stress markers include changes in heart rate, heart rate variability, blood pressure, body temperature, blood glucose, baroreflex sensitivity, among others.

Toluene is a solvent widely produced and used by the industrial sector. Noise is a nuisance present in many professional sectors. We aimed to investigate in the present study the impact of exposure to toluene (300 ppm) and/or noise 85 dB(A) on rat heart and to assess the preventive effect of olive leaf extract OLE (40 mg/Kg/day) administrated for six weeks (5 days/week). Forty-eight male Wistar rats were divided into 8 groups (n = 6/group): C: control group, C+OLE: control group received OLE, N: group exposed to noise, N+OLE: group exposed to noise and received OLE, T: group exposed to toluene, T+OLE: group exposed to toluene and received OLE, NT: group co-exposed to noise and toluene, NT+OLE: group co-exposed to noise and toluene received. At the end of the experimental period, heart and blood samples were collected. Plasma cholesterol (CHOL), triglycerides (TG), lactate dehydrogenase (LDH) and creatine kinase (CK) levels were determined. Histopathological investigation was determined by HE staining. Superoxide dismutase (SOD), catalase (CAT) activities and malondialdehyde (MDA) level in heart tissue were also measured. Our results showed that Simultaneous exposure to noise and toluene altered CHOL, TG, LDH and CK levels. In parallel, an increase in lipid peroxidation levels and Superoxide dismutase activity with a decrease in catalase activity in the heart were shown. A significant alteration in the myocardium was also recorded. OLE treatment modulated theses oxidative and histological changes. In conclusion, OLE was found to correct the deleterious effects induced by combined exposure to noise and toluene.
