Impact and cost‐effectiveness of the national scale‐up of HIV pre‐exposure prophylaxis among female sex workers in South Africa: a modelling analysis

Abstract: Introduction:In 2016, South Africa (SA) initiated a national programme to scale-up pre-exposure prophylaxis (PrEP) among female sex workers (FSWs), with ∼20,000 PrEP initiations among FSWs (∼14% of FSW) by 2020. We evaluated the impact and cost-effectiveness of this programme, including future scale-up scenarios and the potential detrimental impact of the COVID-19 pandemic. Methods: A compartmental HIV transmission model for SA was adapted to include PrEP. Using estimates on self-reported PrEP adherence from a… Show more

“…Counterfactualbased modelling shows that fully meeting HIV prevention and treatment needs of WESW would substantially impact the HIV epidemic in SSA, and that averting HIV transmission associated with sex work should remain a programmatic priority. 6,31 Our study was subject to limitations. Data were available from fewer than half of SSA countries, with WCA particularly under-represented, and estimates for ESA largely from Kenya and Zimbabwe.…”

“…As is in other countries such as South Africa, use of PrEP among female sex workers significantly reduces the costs on management of HIV infections with projections showing that each dollar spent on PrEP among female sex workers results into 1.42 dollars saved on anti-retroviral therapy (ART) costs. 33 We postulate that having symptoms of depression was significantly associated with use of PrEP among female sex workers due to fear of contracting HIV infection and its sequalae, which would worsen the depressive symptoms or result in severe depression. However, this is contrary to the findings from other studies which indicate that depression resulting from factors such as intimate partner violence, client-perpetrated violence, forced condomless sex, and PrEP-associated stigma are associated with decreased use of PrEP.…”
