Immunotherapy for Peritoneal Carcinomatosis: Challenges and Prospective Outcomes

Abstract: Peritoneal metastasis, also known as peritoneal carcinomatosis (PC), is a refractory cancer that is typically resistant to conventional therapies. The typical treatment for PC is a combination of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Recently, research in this area has seen significant advances, particularly in immunotherapy as an alternative therapy for PC, which is very encouraging. Catumaxomab is a trifunctional antibody intraperitoneal (IP) immunotherapy authori… Show more

“…The study demonstrated that delivering anti-PD-L1 Abs to the TAMs in the peritoneal cavity via the NPs platforms resulted in enhanced anti-tumor activity and improved survival in the mouse model [ 2 ]. Although there have been significant advancements in utilizing NPs systems for IMT in the treatment of peritoneal carcinomatosis, it is crucial to gain a more comprehensive understanding of the impact of locoregional therapy on the physiological and immune systems of human hosts [ 136 ].…”

“…Peritoneal carcinomatosis (PC) is a deadly neoplastic condition with a frequently unfavorable prognosis. Characterized by metastatic infiltration of the peritoneum, a serous membrane enveloping the abdominal viscera, this condition poses significant challenges regarding therapeutic interventions and clinical outcomes [ 1 ].…”

“…PIPAC has shown promise in improving drug penetration and distribution in the peritoneum, 10 making it a suitable option for patients who are not candidates for HIPEC. Other therapeutic strategies include palliative care focusing on symptom management, intraperitoneal radiotherapy, 11 and emerging targeted therapies 12 that address specific molecular pathways involved in tumor growth and spread. In cases where PC is limited or resectable, surgical debulking or peritonectomy might be considered to reduce tumor load.…”
