Adipophilin is a monoclonal antibody against a protein on the surface of intracellular lipid droplets, and it was recently shown to be expressed in sebocytes and sebaceous lesions. This study examines adipophilin expression in various sebaceous lesions and other cutaneous tumors with a clear cell histology that may mimic sebaceous differentiation. A total of 117 cutaneous clear cell lesions including 16 sebaceous adenomas, 25 sebaceous carcinomas, 8 basal cell carcinomas, 12 squamous cell carcinomas, 6 xanthomas, 10 xanthelasmas, 10 xanthogranulomas, 4 balloon cell nevi, 5 trichilemmomas, 8 clear cell hidradenomas, and 13 metastatic renal cell carcinomas were examined using immunohistochemistry for the expression of adipophilin. Of these 117 lesions, 42 (36%) were from the periocular region. Adipophilin was expressed in 16 of 16 (100%) sebaceous adenomas, 23 of 25 (92%) sebaceous carcinomas, 10 of 10 (100%) xanthelasmas, 9 of 10 (90%) xanthogranulomas, 6 of 6 (100%) xanthomas, and 9 of 13 (62.5%) metastatic renal cell carcinomas. The characteristic staining pattern differed between sebaceous and non-sebaceous tumors with the former showing a membranous vesicular pattern and the latter being more granular. Adipophilin expression was not seen in any of the other lesions with clear cell histology, basal cell carcinomas, or squamous cell carcinomas, including cases that had focal clear cell differentiation. Adipophilin can be valuable in an immunohistochemical panel when evaluating cutaneous lesions with clear cell histology as it identifies intracytoplasmic lipid vesicles in sebaceous and xanthomatous lesions. In periocular lesions, it is effective in helping to exclude basal cell carcinoma and squamous cell carcinoma when sebaceous carcinoma is under consideration. Adipophilin expression is not as useful for the differential diagnosis that includes metastatic renal cell carcinoma, a rare but important, diagnostic differential. The pattern of adipophilin reactivity is important to observe as membranous vesicular staining is suggestive of intracellular lipids whereas granular cytoplasmic reactivity is not. S inonasal malignant tumors constitute less than 1% of all neoplasms and approximately 3% of those of the upper aerodigestive tract. 1 Sinonasal malignant neoplasms most commonly affect the maxillary sinuses (approximately 60%), followed by the nasal cavity (approximately 22%), ethmoidal sinus (approximately 15%), and frontal and sphenoid sinuses (Ͻ3%). 1 Sinonasal neoplasms are diverse, with most being squamous cell carcinoma or variants (55%), followed by nonepithelial neoplasms (20%), glandular tumors (15%), undifferentiated carcinoma (7%), and miscellaneous tumors (3%). 1 We describe an exceedingly unusual case of ethmoidal lipid-rich carcinoma with focal glandular structures. To our knowledge, such a case has not been documented in the literature to date. REPORT OF A CASEA 75-year-old Japanese woman with a 1-month history of left-sided yellowish rhinorrhea, nasal epistaxis, and nasal stuffiness was admitted to our hospital. Her family history was unremarkable and there was no occupational history of exposure to wood dust. Computed tomography and magnetic resonance imaging showed a 48ϫ11-mm tumorous lesion located in the left anterior ethmoidal sinuses and invading the left middle nasal meatus, the inferior wall of the left orbit, and the left anterior orbital contents. Fluid accumulation was evident in the left maxillary and frontal sinuses, but there was no radiological evidence of intracranial extension (Figure 1). Positron emission tomography showed fludeoxyglucose F 18 uptake with a maximum standard uptake value of 11.0 in the area corresponding to the left ethmoidal lesion. Bone scintigraphy detected strong accumulation in the left maxilla. Neither distant metastasis nor lymph node metastasis was identified (In-ternational Union Against Cancer classification, T4aN0M0). The serum levels of tumor markers, including squamous cell carcinoma-related antigen, CYFRA 21-1, and carcinoembryonic antigen, were within reference ranges. Nasoscopy detected a yellowish and hemorrhagic tumor on the left middle nasal meatus, and a biopsy specimen demonstrated the presence of carcinoma. We subsequently performed exploratory surgery of the left paranasal sinuses and confirmed that the tumor had originated in the left anterior ethmoidal sinuses. We performed a partial tumorectomy of the ethmoidal lesion to confirm the histopathological diagnosis and inserted a catheter into the left superficial temporal artery for use during chemotherapy. Although the patient subsequently underwent chemotherapy with a combination of cisplatin, pirarubicin, and cyclophosphamide, the tumor did not respond to 1 cycle of chemotherapy and the patient transferred to another hospital.The 23ϫ 8-mm tissue specimen taken from the left ethmoidal tumor was prepared for histopathologic examination. A hallmark of cancer cells is their remarkable ability to efficiently adapt to favorable and hostile environments. Due to a unique metabolic flexibility, tumor cells can grow even in the absence of extracellular nutrients or in stressful scenarios. To achieve this, cancer cells need large amounts of lipids to build membranes, synthesize lipid‐derived molecules, and generate metabolic energy in the absence of other nutrients. Tumor cells potentiate strategies to obtain lipids from other cells, metabolic pathways to synthesize new lipids, and mechanisms for efficient storage, mobilization, and utilization of these lipids. Lipid droplets (LDs) are the organelles that collect and supply lipids in eukaryotes and it is increasingly recognized that the accumulation of LDs is a new hallmark of cancer cells. Furthermore, an active role of LD proteins in processes underlying tumorigenesis has been proposed. Here, by focusing on three major classes of LD‐resident proteins (perilipins, lipases, and acyl‐CoA synthetases), we provide an overview of the contribution of LDs to cancer progression and discuss the role of LD proteins during the proliferation, invasion, metastasis, apoptosis, and stemness of cancer cells.
