Summary Type 2 diabetes is a global health priority, given that it is driven, in part, by an ageing population, the role of immune senescence has been overlooked. This is surprising, as the functional impairments of senescent T cells show strong similarities to patients with hyperglycaemia. Immune senescence is typified by alterations in T cell memory, such as the accumulation of highly differentiated end‐stage memory T cells, as well as a constitutive low‐grade inflammation, which drives further immune differentiation. We show here in a preliminary study that people living with type 2 diabetes have a higher circulating volume of senescent T cells accompanied with a higher level of systemic inflammation. This inflammatory environment drives the expression of a unique array of chemokine receptors on senescent T cells, most notably C‐X‐C motif chemokine receptor type 2. However, this increased expression of migratory markers does not translate to improved extravasation owing to a lack of glucose uptake by the T cells. Our results therefore demonstrate that the presence of senescent T cells has a detrimental impact on immune function during type 2 diabetes.

Perturbations in CD4(+) and CD8(+) T-cell phenotype and function are hallmarks of tuberculosis-diabetes co-morbidity. However, their contribution to the pathogenesis of this co-morbidity and the effect of anti-tuberculosis treatment on the phenotype of the T-cell subsets is poorly understood. In this study, we examined the frequency of different T-cell subsets in individuals with pulmonary tuberculosis (PTB) with diabetes mellitus (DM) or without coincident diabetes mellitus (NDM) before, during and after completion of anti-tuberculosis chemotherapy. PTB-DM is characterized by heightened frequencies of central memory CD4(+) and CD8(+) T cells and diminished frequencies of naive, effector memory and/or effector CD4(+) and CD8(+) T cells at baseline and after 2 months of treatment but not following treatment completion in comparison with PTB-NDM. Central memory CD4(+) and CD8(+) T-cell frequencies exhibited a positive correlation with fasting blood glucose and glycated haemoglobin A1c levels, whereas the frequencies of naive and effector memory or effector CD4(+) and CD8(+) T cells exhibited a negative correlation. However, the frequencies of CD4(+) and CD8(+) T-cell subsets in individuals with PTB exhibited no significant relationship with bacterial burdens. Finally, although minor alterations in the T-cell subset compartment were observed at 2 months of treatment, significantly decreased frequencies of central memory and significantly enhanced frequencies of naive CD4(+) and CD8(+) T cells were observed at the completion of treatment. Our data reveal a profound effect of coexistent diabetes on the altered frequencies of central memory, effector memory and naive T cells and its normalization following therapy.

Через 1 год условно-защитные титры антител (> 1:40) Дети с сахарным диабетом 1-го типа (СД1) относятся к группе высокого риска тяжелого течения гриппа. Цель исследо-вания: изучить иммуногенность и безопасность трехвалентной иммуноадъювантной субъединичной противогриппозной вакцины у детей с СД1. Методы. В проспективное исследование с сентября по ноябрь 2014 г. включали детей с СД1 в воз-расте от 3 до 17 лет, вакцинированных против гриппа трехвалентной иммуноадъювантной субъединичной противогрип-позной вакциной и невакцинированных вследствие отказа родителей. Антитела к вирусу гриппа определяли с помощью ОБОСНОВАНИЕПроблема предотвращения вирусных инфекций у детей продолжает волновать медицинское сообщество. Особое внимание уделяется профилактике гриппоз-ной инфекции, которую не удается контролировать, несмотря на ежегодную предэпидемическую подготовку. Панде мия гриппа 2009 г. и постпандемический период в очередной раз доказали, что вакцинопрофилактика остается самой эффективной и безопасной мерой защи-ты против гриппа как в отношении тяжести течения, так и предупреждения развития заболевания: так, в работе Л. С. Карповой и соавт. сообщалось, что среди умерших от гриппа детей практически не было привитых [1].Пациенты с хроническими заболеваниями по-преж-нему составляют группу риска по тяжелому течению грип-па. Новый пандемический штамм А/H1N1/pdm09 спо-собствовал утяжелению течения гриппа и развитию опасных осложнений у пациентов с хроническими забо-леваниями респираторного тракта, сахарным диабетом, а также в группе беременных [1][2][3]. Следует отметить, что после пандемии гриппа 2009 г. среди умерших продолжа-ет увеличиваться доля лиц, имеющих нарушение обмена веществ, включая сахарный диабет (СД) [1,4]. Именно поэтому Всемирная организация здравоохранения (ВОЗ) указывает на необходимость расширения охвата привив-ками против гриппа населения из групп риска [5]. Оригинальная статьяНа современном этапе во всем мире отработана практика ежегодной вакцинации против гриппа паци-ентов с СД [6]. При этом считается, что иммунопрофи-лактика больных СД 1-го и 2-го типов (СД1, СД2) должна стать такой же рутинной процедурой, как и определение гликозилированного гемоглобина [7].
