Two histological classifications are used for this purpose: World Health Organization (WHO) classification, based on cytological atypia, disregarding glandular complexity, and endometrial intraepithelial neoplasia (EIN) classification, based on several different parameters. B-cell lymphoma 2 (Bcl-2) loss has been studied as immunohistochemical marker with the aim of improving the differential diagnosis between benign and premalignant hyperplasia. We aimed to evaluate: (A) Bcl-2 loss as marker of endometrial precancer, by assessing it in proliferative endometrium, benign hyperplasia, premalignant hyperplasia, and endometrial cancer; (B) the diagnostic accuracy of Bcl-2 in the differential diagnosis between benign and premalignant endometrial hyperplasia; (c) how the results change according to the histological classification and the thresholds of Bcl-2 expression used. Material and methods: Electronic databases were searched from their inception to March 2018. All studies assessing Bcl-2 immunohistochemistry in endometrial specimens were included.Results: In total, 20 observational studies assessing 1,278 specimens were included.Bcl-2 loss rates were not significantly different between proliferative endometrium and benign hyperplasia (P = 0.12) and between premalignant hyperplasia and endometrial cancer (P = 0.53). Among hyperplasias, Bcl-2 loss was significantly associated with premalignancy, according to both the WHO (OR = 4.39; P < 0.00001) and EIN classifications (OR = 6.07; P = 0.01), and also with architecture complexity (OR = 2.06; P = 0.02). Using the WHO classification, Bcl-2 loss showed low diagnostic accuracy in detecting premalignant hyperplasia (area under the curve [AUC] = 0.708), with a sensitivity of 0.41, a specificity of 0.81, a positive likelihood ratio of 3.22, and a negative likelihood ratio of 0.69. Purpose: We aimed to compare expression levels of anti-apoptotic and proapoptotic genes in parametrium, sacrouterine and round ligaments of women without and sign of pelvic organ prolapse (POP) with respect to menopausal status. We hypothesized that apoptosis related gene expressions in female pelvic tissues may be altered during menopause. Methods: The study groups consisted of women who did not have POP symptoms pre-menopausal (n=10) and menopausal (n=10). Three different types of tissue samples (Parametrium-Round Ligament-Sacrouterine Ligament) were taken from the cases and RNA was isolated from these tissues. After purifying and quantifying RNA samples, qPCR was used to determine the expression levels of anti-apoptotic and pro-apoptotic genes. Results: BCL-2 gene expression levels were significantly lower in all tissues of menopausal patients compared to all tissues of premenopausal patients. Sacrouterine ligament tissue BAD expression level was significantly high (p= 0.035), BCL-2/BAD ratio was significantly lower in menopausal patients compared to premenopausal patients (p = 0.006). Conclusions: Apoptosis-related protein levels change during menopause, pro-apoptotic gene expressions decrease and anti-apoptotic gene expressions increase. Significant alteration of BCL-2 and BAD expression in sacrouterine ligament with respect to menopausal status was observed, suggesting that sacrouterine ligament which plays a crucial role for restoring normal pelvic anatomy and support for genital organs, could be more affected from menopause rather than other pelvic tissues.

Introduction: Biomaterial technology due to its lack of or minimal side effects in tissues has great potential. Traditionally biomaterials used were cobalt-chromium, stainless steel and nitinol alloys. Biomaterials such as magnesium (Mg) and zinc (Zn) have good biocompatibility and consequently can be a potential material for medical implants. To date, the effects of AZ31 alloy stent on cell apoptosis are still unclear. The current investigation was designed to determine the effect of AZ31 alloy stent on necrosis and apoptosis of common bile duct (CBD) epithelial cells. Material and methods: We experimented with application of different concentrations of AZ31 alloy stent to primary mouse extrahepatic bile epithelial cells (MEBECs) and estimated the effect on apoptosis and necrotic cells. Apoptosis and pro-apoptosis expression were estimated through real-time PCR. For in vivo protocol, we used rabbits, implanted the AZ31 bile stent, and estimated its effect on the CBD. AZ31 (40%) concentration showed an effect on the apoptotic and necrotic cells. Results: Real-time PCR revealed that AZ31 (40%) concentration increased the apoptotic genes such as NF-κB, caspase-3, Bax and Bax/Bcl-2 ratio as compared to the control group. In the in vivo experiment, AZ31 alloy stents were implanted into the CBD and showed an effect on the alteration the hema tological, hepatic and non-hepatic parameters. Conclusions: To conclude, it can be stated that AZ31 induces apoptosis via alteration in genes including nuclear factor kappa-B (NF-κB), caspase-3, Bax and Bax/Bcl-2 ratio and improved the hematological, hepatic and non-hepatic parameters.
