The morphological features of 32 cases of malignant lymphomas involving the central nervous system presenting over a 32 year period were reviewed and the lymphomas redefined using current classifications. Twenty-four cases (75%) of non-Hodgkin's lymphomas were reclassified using the Kiel classification. There were 18 low grade non-Hodgkin's lymphomas (comprising 11 lymphoplasmacytoid, five lymphocytic and two centroblastic-centrocytic) and six high grade tumours (comprising two centroblastic, two immunoblastic, one unclassifiable and one lymphoblastic lymphoma). Cytologically the great majority of non-Hodgkin's lymphomas were B-cell lymphomas. The eight cases (25%) of Hodgkin's disease were classified by the Rye subtype and consisted of three mixed cellularity, two lymphocyte depletion, two lymphocyte predominant and one nodular sclerosis. The presence of intracytoplasmic immunoglobulins as well as markers for histiocytic cells were studied by the immunoperoxidase technique using polyclonal antisera. A monoclonal staining pattern, as revealed by light chain restriction, was found in nine cases (38%) of the non-Hodgkin's lymphomas confirming their B-cell origin. With the Marshall's metalophil method and the other histiocytic markers, scattered reactive microglial cells and histiocytic reticulum cells were found throughout the tumours in most cases. No histiocytic lymphomas were present in the series.

We present a 31-year-old female patient with primary non-Hodgkin's lymphoma of the CNS after immunosuppressive therapy. Colitis ulcerosa had been diagnosed 2 years previously. Prophylactic therapy with azathioprine over 9 months was stopped after the development of listeria meningitis which was treated successfully with antibiotics. At this time native CCT was normal. Three months later the patient developed an epileptic seizure and multiple cerebral lesions were detected in CCT and MRI. Although antibiotic therapy was started, the cerebral lesions showed no regression. Stereotactic biopsy revealed immunochemical and histologic high-grade malignant B cell lymphoma. The risk of primary CNS lymphoma under azathioprine treatment for an autoimmune disease with a possible congenital immunodeficiency is presented and the literature is reviewed.

Primary central nervous system lymphomas (PCNSLs) are predominantly high-grade diffuse large B-cell lymphomas. Incidence has been rising in immunocompetent as well as in immunocompromised patients, but main characteristics like age of onset, sex distribution, CT/MRI appearance and localization, molecular biology and morphology are different in both groups. These characteristics explain only in part the poor prognosis of the latter group. In today’s neurosurgical practice PCNSL is regarded as a special entity among malignant brain tumors. This is due to the fact that surgical resection is no longer the treatment of choice ever since successful radio-/chemotherapy regimens have been introduced. Thus, in cerebral lesions suspicious of PCNSL, the histopathological diagnosis should be established by CT- or MRI-guided stereotactic biopsy or by CSF cytology. One should abstain from corticosteroid administration before stereotactic biopsy as this would mask the tumor’s morphological appearance and possibly render an accurate histological diagnosis impossible. Postoperative tumor staging depends on the favored theory of histiogenesis. There is still a controversial discussion about the need to exclude systemic disease. Absolutely mandatory are a full clinical examination, enhanced CT or MRI images from the brain, CSF cytology, bone marrow biopsy, slit-lamp examination, and an HIV test. Today’s therapy consists of corticosteroids and a methotrexate-based chemotherapy with adjuvant radiotherapy. As an exception, cyclophosphamide, methotrexate, procarbacine and dexamethasone (CMPD) administered after blood-brain barrier disruption with mannitol are at least as effective as multimodality treatment with methotrexate plus radiation therapy. More aggressive regimens did not prove superior regarding survival but result in a higher therapy-related complication rate.
