Immunisation of neonates at high risk of hepatitis B in England and Wales: national surveillance.

Abstract: IntroductionThe earlier in life that infection with hepatitis B virus occurs the greater the possibility that it will lead to persistent carriage, which greatly increases the risk of cirrhosis or primary hepatic carcinoma in later life. 2 More than 80% of infants whose mothers are carriers of the hepatitis B surface antigen and also have the hepatitis B e antigen (a serological marker of high infectivity) become persistent carriers. This outcome is less common when the mother is negative for the hepatitis B e … Show more

“…On the other hand 27 of 30 (90%) examined for anti-HBs were positive 2-6 years after the last dose of vaccine, and the three anti-HBs negatives were also HBsAg negative. This seroconversion rate is similar to that of other published studies [Stevens et al, 1985;Wong et al, 1984;Polakoff and Vandervelde 1990;Okun et al, 19901. Six of our infants received vaccine only; three of three tested were HBsAg negative, and two of two tested were without antibodies.…”

“…5 In an earlier study for the Public Health Laboratory, 89 of 102 babies from an original cohort of 155 received three doses of vaccine 7. In the Netherlands, where there is a centrally organised service with different vaccination schedules, uptake of three doses of vaccine rose from 80% to 94% between 1989 and 1991 6.…”

“…This coverage was achieved in an inner city area where routine immunisation uptake, for example, DPT and MMR is traditionally poor7 8; uptake of third DPT was 91% and MMR 85% in Hackney in 1995. In contrast to other studies, uptake of hepatitis B immunisation was greater overall than that of routine immunisations 4…”
