Immune response to Haemophilus influenzae type b conjugate vaccine in preterm infants

Abstract: There is a possibility that perinatal factors and the environment unique to preterm infants are related to their lower antibody positivity rates compared to full-term infants. It may therefore be preferable to modify the proposed immunization schedule.

“…Consistent with the mostly anti-inflammatory response to innate immune stimulation in premature infants, impaired responses to vaccines given around birth have been noted (Baxter, 2010; D’Angio, 2007; Tsuda et al, 2011). However, TLR-mediated cytokine responses of preterm infants change over the first year of life.…”

“…Table 1 provides clues on the short-term effects of glucocorticoid exposure on immunity. With few exceptions [112][113][114][115], fetal cord blood was employed to measure parameters such as cytokine levels, cell counts or leukocyte function, which we classified into innate or adaptive immunity (refer to Table 1 for references). Hampered by the considerable heterogeneity in immune parameters between individuals [116], differences in the selected readouts and methodologies employed, and the small size of these studies, the collective data are conflicting and inconclusive.…”

“…However, antenatal corticosteroid therapy was also associated with increased responses to Haemophilus influenzae type b [112], underscoring the need for more investigation on the role of prenatal glucocorticoids on humoral immune responses.…”

“…Immunization of preterm infants from Poland and Spain with a 13-valent pneumococcal conjugate vaccine at the ages of 2, 3, 4 and 12 months led to lower but acceptable antibody levels (11). Based on the response to Haemophilus influenzae type b (Hib) vaccine, schedule modification (12) and need for boosters (13) were recommended in Japan and Poland, respectively. In Bangladesh, hepatitis B vaccine (Hep-B) generated protective, but, <100 mIU/ml anti-HBs titers in 26.4% Preterm infants (14).…”
